目的骨性关节炎( osteoarthritis,OA )是一种退行性骨关节病,该病病因和发病机制尚不明确。微量元素能通过参与人体内的新陈代谢、各种生物和化学反应等,维持机体正常生理活动,如果体内各种微量元素含量失调,就有可能引发多种疾病。本研究通过测定骨性关节炎滑膜组织中B、Mg、Al、K、Ca、Ti、Cr、Mn、Fe、Ni、Cu、Zn、Se、Sr、Ba、Pb 等16种元素含量,探讨 OA 患者滑膜组织中这些元素的水平和疾病发生、发展的关系,为 OA 的机制研究和诊治提供相关的理论依据。方法选择骨性关节炎患者为实验组,单纯外伤引起的半月板损伤患者为对照组。术中采集患者滑膜组织样本,经微波消解处理,用电感耦合等离子体质谱( ICP-MS )测定组织中16种元素含量,数据经 SAS9.0软件统计分析。结果实验组滑膜组织中硒( Se )、铁( Fe )、锌( Zn )、铜( Cu )、硼( B )、钙( Ca )和钾( K )7种元素含量与对照组比较,差异均有统计学意义( P<0.05),其中实验组滑膜组织中 Fe、B、Se 元素含量低于对照组,而 Ca、K、Cu、Zn 元素的含量高于对照组;锰( Mn )、铬( Cr )、锶( Sr )、镍( Ni )、钡( Ba )、铝( Al )、铅( Pb )、锑( Sb )、镁( Mg )元素含量没有明显变化。结论骨性关节炎患者滑膜中硒、铁、锌、铜、硼、钙和钾7种元素含量变化明显,这些元素都是机体的必需元素,它们可能通过影响蛋白质、酶的代谢导致骨性关节病的发生。

목적골성관절염( osteoarthritis,OA )시일충퇴행성골관절병,해병병인화발병궤제상불명학。미량원소능통과삼여인체내적신진대사、각충생물화화학반응등,유지궤체정상생리활동,여과체내각충미량원소함량실조,취유가능인발다충질병。본연구통과측정골성관절염활막조직중B、Mg、Al、K、Ca、Ti、Cr、Mn、Fe、Ni、Cu、Zn、Se、Sr、Ba、Pb 등16충원소함량,탐토 OA 환자활막조직중저사원소적수평화질병발생、발전적관계,위 OA 적궤제연구화진치제공상관적이론의거。방법선택골성관절염환자위실험조,단순외상인기적반월판손상환자위대조조。술중채집환자활막조직양본,경미파소해처리,용전감우합등리자체질보( ICP-MS )측정조직중16충원소함량,수거경 SAS9.0연건통계분석。결과실험조활막조직중서( Se )、철( Fe )、자( Zn )、동( Cu )、붕( B )、개( Ca )화갑( K )7충원소함량여대조조비교,차이균유통계학의의( P<0.05),기중실험조활막조직중 Fe、B、Se 원소함량저우대조조,이 Ca、K、Cu、Zn 원소적함량고우대조조;맹( Mn )、락( Cr )、송( Sr )、얼( Ni )、패( Ba )、려( Al )、연( Pb )、제( Sb )、미( Mg )원소함량몰유명현변화。결론골성관절염환자활막중서、철、자、동、붕、개화갑7충원소함량변화명현,저사원소도시궤체적필수원소,타문가능통과영향단백질、매적대사도치골성관절병적발생。
Objective Osteoarthritis ( OA ) is one kind of degenerative osteoarthropathy. The pathogenesis of OA is not clear, and the studies about it are rare. Trace elements in human body can maintain body’s normal physiological activity by the process of metabolism, various biological and chemical reaction and so on. If the content of trace elements of body is in disorder, it may cause a variety of diseases. This experiment analyses the distribution of the 16 elements ( B, Mg, Al, K, Ca, Ti, Cr, Mn, Fe, Ni, Cu, Zn, Se, Sr, Ba, Pb ) in the synovial membrane of the patient with OA, and to discuss the regularity of distribution of the elements and their effects on the etiology and pathogenesis of OA. It will provide the basis in theory for the clinical diagnosis and etiopathogenesis. Methods Patients of osteoarthritis and meniscus injury were considered to be the object of the study. Patients with OA were of experimental group. Patients with meniscus injury caused by simple trauma were of control group. We collected the synovial membrane from the patients through operation and processed the samples with microwave digestion. After that, Inductively Coupled Plasma Mass Spectrometry ( ICP-MS ) method was established for the determination of 16 elements. The experiment results were analyzed by SAS 9.0. Results The concentrations of 7 trace elements such as Fe, Se, B, Ca, K, Cu and Zn were significant different between the synovial membrane of osteoarthritis and the normal ( P<0.05 ). The concentrations of Fe, Se, B in the synovial membrane of osteoarthritis were lower than that in the side of normal. The concentrations of Ca, K, Cu, Zn in the synovial membrane of osteoarthritis were higher than that in the side of normal. No significant change occurred in the concentration of Mn, Cr, Sr, Ni, Ba, Al, Pb, Sb, Mg. Conclusions The concentrations of 7 trace elements such as Fe, Se, B, Ca, K, Cu and Zn in the synovial membrane of patients of osteoarthritis changes obviously. These elements are the essential elements of body, which probably lead to osteoarthritis by affecting the metabolism of protein and enzyme.
