目的对全机器人心脏手术后早期患者的疼痛特点进行观察,找出疼痛规律,制定有效的干预措施。方法通过应用视觉模拟评分法对289例机器人心脏手术患者术后第1~4日内的疼痛程度进行评估,并分析疼痛的原因。结果手术后第1、4天轻度疼痛患者所占比重高,分别为81.0%和67.8%;第2、3天中度以上疼痛患者增加,分别为51.9%和50.5%,剧烈疼痛分别为7.3%和2.8%。在引起疼痛的原因中,手术伤口造成的疼痛占主要原因,术后第2、3日分别为96.5%和84.1%,术后第1、4日所占比重也仍占各类原因之首,其次是胸部引流管牵拉、体位的改变、肺部物理治疗护理及肩背痛等。结论全机器人心脏手术后疼痛是有规律可循的,应采取针对性的护理措施,减轻患者的疼痛。

목적대전궤기인심장수술후조기환자적동통특점진행관찰,조출동통규률,제정유효적간예조시。방법통과응용시각모의평분법대289례궤기인심장수술환자술후제1~4일내적동통정도진행평고,병분석동통적원인。결과수술후제1、4천경도동통환자소점비중고,분별위81.0%화67.8%;제2、3천중도이상동통환자증가,분별위51.9%화50.5%,극렬동통분별위7.3%화2.8%。재인기동통적원인중,수술상구조성적동통점주요원인,술후제2、3일분별위96.5%화84.1%,술후제1、4일소점비중야잉점각류원인지수,기차시흉부인류관견랍、체위적개변、폐부물리치료호리급견배통등。결론전궤기인심장수술후동통시유규률가순적,응채취침대성적호리조시,감경환자적동통。
Objective To study the regularity of pain in patients after total robotic cardiac surgery by observing its characteristics and formulate its effective intervention measures. Methods The pain in 289 patients on days 1-4 was assessed according to the visual analogue scale. The causes of pain were analyzed. Results The percentage of patients with mild pain on days 1 and 4, moderate pain on days 2 and 3, and severe pain was 81.0%and 67.8%, 51.9%and 50.5%, and 7.3%and 2.8%, respectively. The most common cause of pain was surgical wound (accounting for 96.5%and 84.1%, respectively, on days 2 and 3 after operation), followed by intra-thoracic drain tube traction, body position change, lung physical therapy, shoulder and back pain. Conclusion The regularity of pain can be observed in patients after total robotic cardiac surgery and nursing measures should be taken to alleviate the pain in different individuals.
