目的:观察VSD联合钢板内固定治疗胫腓骨开放性骨折的临床效果。方法:选取本科2009年2月-2012年6月收治的30例胫腓骨开放性骨折患者,按照Gustilo分型,Ⅱ型骨折20例,ⅢA型骨折8例,ⅢB型骨折1例,ⅢC型骨折1例,均急诊行清创、VSD联合钢板内固定治疗,持续负压引流7~10 d,伤口肉芽组织生长良好后行植皮或皮瓣修复创面。结果:随访12~24个月,创面中完全愈合30例,无感染病例,骨折骨性愈合28例,骨折延迟愈合2例,再行植骨术,无截肢病例。结论:VSD能够促进肉芽组织的生长,加速创面的愈合,降低伤口的感染率,缩短治疗时间及减轻患者经济负担等优点,彻底清创后,采用VSD联合钢板内固定治疗胫腓骨开放性骨折是一种安全有效的治疗方法。
目的:觀察VSD聯閤鋼闆內固定治療脛腓骨開放性骨摺的臨床效果。方法:選取本科2009年2月-2012年6月收治的30例脛腓骨開放性骨摺患者,按照Gustilo分型,Ⅱ型骨摺20例,ⅢA型骨摺8例,ⅢB型骨摺1例,ⅢC型骨摺1例,均急診行清創、VSD聯閤鋼闆內固定治療,持續負壓引流7~10 d,傷口肉芽組織生長良好後行植皮或皮瓣脩複創麵。結果:隨訪12~24箇月,創麵中完全愈閤30例,無感染病例,骨摺骨性愈閤28例,骨摺延遲愈閤2例,再行植骨術,無截肢病例。結論:VSD能夠促進肉芽組織的生長,加速創麵的愈閤,降低傷口的感染率,縮短治療時間及減輕患者經濟負擔等優點,徹底清創後,採用VSD聯閤鋼闆內固定治療脛腓骨開放性骨摺是一種安全有效的治療方法。
목적:관찰VSD연합강판내고정치료경비골개방성골절적림상효과。방법:선취본과2009년2월-2012년6월수치적30례경비골개방성골절환자,안조Gustilo분형,Ⅱ형골절20례,ⅢA형골절8례,ⅢB형골절1례,ⅢC형골절1례,균급진행청창、VSD연합강판내고정치료,지속부압인류7~10 d,상구육아조직생장량호후행식피혹피판수복창면。결과:수방12~24개월,창면중완전유합30례,무감염병례,골절골성유합28례,골절연지유합2례,재행식골술,무절지병례。결론:VSD능구촉진육아조직적생장,가속창면적유합,강저상구적감염솔,축단치료시간급감경환자경제부담등우점,철저청창후,채용VSD연합강판내고정치료경비골개방성골절시일충안전유효적치료방법。
Objective:To observe the clinical effect by VSD combine with plate internal fixation for open fractures of tibia and fibula.Method:30 cases of patients were treated with open fractures of tibia and fibula were selected from February 2009 to June 2012,20 patients of typeⅡ,8 patients of typeⅢA,1 patient of typeⅢB,1 patient of typeⅢC(Gustilo classification).All patients were treated by VSD combined with internal fixation after debridement,and were continuing attracted for 7 to 10 days using vacuum sealing drainage,when granulation tissue grew well,skin grafting or skin flap repairing was done.Result:All cases were followed up for 12 to 24 months,30 cases of wound completely healed without infection,28 cases of patients with fracture bone healing,2 cases delayed healing,further bone grafting,no amputations.Conclusion:The vacuum assisted closure technology can promote the growth of granulation,promote wound healing,reduce wound infection rate,shorten the course of the disease,and reduce health care cost, etc.After thorough debridement,VSD combined with internal fixation is a safe and effective method to treat open fracture of tibia and fibula.
