目的：探讨帕瑞昔布钠超前镇痛对全髋关节置换术后患者疼痛的影响。方法便利抽样法选择2011年10月至2012年1月在上海市东方医院骨科实施单侧全髋关节置换术的30例患者为对照组，采用骨科常规围术期护理；同法选择2012年2月至2012年6月实施单侧全髋关节置换术的30例患者为观察组，围术期采用帕瑞昔布钠超前镇痛护理，比较两组患者术后不同时间的疼痛数字等级评定量表（numerical rating scale ，NRS）的评分、有无不良反应及患者满意度。结果观察组术后患者1、6、12、24 h的N RS疼痛评分均低于对照组，差异均有统计学意义（ P＜0．05或 P＜0．01）；观察组术后患者48 h静息时的NRS疼痛评分与对照组比较，差异无统计学意义（P＞0．05）。两组患者术后不良反应的发生情况的差异均无统计学意义（均 P＞0．05）。观察组患者的满意度高于对照组，差异有统计学意义（P＜0．05）。结论帕瑞昔布钠超前镇痛能有效缓解术后患者疼痛，降低不良反应发生率，使患者及早开始行功能锻炼，提高患者的满意率。
목적：탐토파서석포납초전진통대전관관절치환술후환자동통적영향。방법편리추양법선택2011년10월지2012년1월재상해시동방의원골과실시단측전관관절치환술적30례환자위대조조，채용골과상규위술기호리；동법선택2012년2월지2012년6월실시단측전관관절치환술적30례환자위관찰조，위술기채용파서석포납초전진통호리，비교량조환자술후불동시간적동통수자등급평정량표（numerical rating scale ，NRS）적평분、유무불량반응급환자만의도。결과관찰조술후환자1、6、12、24 h적N RS동통평분균저우대조조，차이균유통계학의의（ P＜0．05혹 P＜0．01）；관찰조술후환자48 h정식시적NRS동통평분여대조조비교，차이무통계학의의（P＞0．05）。량조환자술후불량반응적발생정황적차이균무통계학의의（균 P＞0．05）。관찰조환자적만의도고우대조조，차이유통계학의의（P＜0．05）。결론파서석포납초전진통능유효완해술후환자동통，강저불량반응발생솔，사환자급조개시행공능단련，제고환자적만의솔。
Objective To observe the effect of parecoxib preemptive analgesia in total hip arthroplasty sur-gery .Methods By convenience sampling ,60 patients were randomly selected and assigned into two groups .The patients in control group (n=30) received the routine nursing measures of orthopedics depart-ment ,and those in observation group(n=30) were accepted parecoxib preemptive analgesia before surgery . Postoperative analgesia effect was assessed by numerical rating scale(NRS) at different time after surgery . Adverse effects and patient satisfaction were also evaluated .Results NRS was lower in control group at 1 , 6 ,12 ,24 hour after surgery than which in observation group (P<0 .05 or P<0 .01 ) ,while there is no sig-nificant difference of the NRS scores between two groups after 48 hours of the surgery(P>0 .05) .The postoperative adverse effects had no statistical significance between two groups as well(P> 0 .05) .The overall satisfaction level of analgesic therapy was significantly higher in observation group(P<0 .05) .Con-clusion Parecoxib preemptive analgesia can effectively relieve the pain and reduce the incidence of adverse effects ,and help patients to start functional exercise as soon as possible and improve their satisfaction .
