目的探讨1.5TMR扩散加权成像(DWI)优化b值在颈髓病变中的检出率的价值。方法收集我院疑有或确诊颈髓病变患者60例行MRI及DWI检查,b值取0、300、500、1000s/mm2。计算DWI的病灶检出率,测量颈段脊髓表观扩散系数( ADC)值并分析各组DWI图像及ADC图像质量,对比不同b值对成像效果的影响。结果 DWI对病变的检出率为95.8%(57/60),60例受检者中57例均获得较满意的DWI和ADC图像。随着b值由0 s/mm2升高至1000 s/mm2,颈髓与脑脊液信噪比逐渐降低,颈髓脑脊液对比噪声比b值为500 s/mm2时最高,此后逐渐降低,且ADC参考范围较稳定。结论b值为500 s/mm2时,颈段脊髓DWI图像质量最好,可以获得较满意的DWI及ADC图像,病变的检出率最高。

Objective Discuss MR 1.5T dif usion weighted imaging (DWI) can optimize the b value in the value of the detection rate of cervical spinal lesions. Methbods Collect our suspected or confirmed cervical spinal lesions of 60 patients with routine MRI and DWI examination, can b value= 0, 300, 500, 300 s/was. Calculating DWI lesion detection rate, and can measure the neck section of the spinal cord apparent diffusion coef icient (ADC) value and analyze each group DWI images and ADC can image quality, contrast dif erent b values on the imaging ef ect. Results DWI for can lesion detection rate was 95.8% (57/60), 60 cases of client in 57 cases obtained satisfactory DWI and ADC can image. As the 0 s/b value was increased to 1000 s/was that the spinal cord and cerebrospinal fluid signal-to-noise ratio decreases, spinal cerebrospinal fluid contrast noise ratio b value=500 s/mm2 was the highest, then gradual y decreased, and the ADC reference range is relatively stable. Conciusion Value of 500 b/s was, the neck section of the spinal cord DWI image quality is best, can get satisfactory DWI and ADC can image, lesion detection rate is highest.
