目的采用 Meta 分析的方法探讨胎儿心内强回声光斑与21-三体综合征的关系。方法双人独立全面检索 CNKI、万方、Pubmed 3个数据库,对符合纳入标准的文献提取研究者姓名,研究时间,效应值等指标,采用 Stata 11.0合并效应值进行发表偏倚检验。结果与正常胎儿相比21-三体综合征胎儿心内强回声光斑的检出率明显增加 OR=7.31,95%CI 5.55~9.63,而孤立出现的胎儿心内强回声光斑并不能增加21-三体综合征的发病风险 OR=1.56,95%CI 0.81~3.01。结论伴有其他21-三体综合征高危因素的胎儿心内强回声光斑孕妇应尽早行羊膜腔穿刺,确定有无21-三体综合征,孤立出现的胎儿心内强回声光斑不伴有其他高危因素时对21-三体综合征无诊断价值。

Objective To analyze the relationship between echogenic intracardiac foci (EIF) and down syndrome. Methods Independent literature review by two authors for those literatures meeting the inclusion criteria in CNKI, Wangfang Data and Pubmed was conducted and the first author name, publication time and OR or RR were abstracted respectively. Stata 11.0 was used for calculating the pooled ORs (95% CI) as well as the publication bias test. Results Compared with normal fetus, the odds ratio of EIF for fetus with down syndrome was 7.31 and 95% CI was 5.55~9.63, however, for fetus with isolated EIF the odds ratio is not significant (OR=1.56, 95% CI=0.81~3.01). Conclusion For EIF fetus with other risk factors of down syndrome, it is better to carry out amniocentesis as soon as possible to confirm whether down syndrome exist, but for isolated EIF there is no diagnostic value for down syndrome.
