目的探讨血清胱抑素C(CysC)在糖尿病肾病(DN)诊断中的意义。方法将95例2型糖尿病患者根据24 h尿微量蛋白排泄率(UAER)水平分为正常蛋白尿组、微量蛋白尿组、临床蛋白尿组,选取同期的35名健康人员为正常对照组,检测4组人员的血清CysC浓度并进行比较。结果与正常对照组(0.55±0.16)mg/L相比,正常蛋白尿组、微量蛋白尿组、临床蛋白尿组的CysC浓度均明显升高[分别为(0.73±0.18)mg/L、(1.21±0.25)mg/L、(1.87±0.24)mg/L](均P<0.01),且3组间的CysC水平差异有统计学意义(P<0.01),同时,微量蛋白尿组与临床蛋白尿组血清CysC浓度与尿微量蛋白排泄率(UAER)呈显著正相关(r分别为0.083,0.675,P<0.01)。结论血清CysC是反映糖尿病肾病患者肾功能的理想指标,对诊断糖尿病肾病早期肾损伤有重要的意义。

Objective To discuss the signiifcance of serum cystatin C ( CysC ) in the diagnosis of diabetic nephropathy ( DN ). Methods Ninety-ifve cases of type 2 diabetic patients according to 24 h urinary protein excretion rate ( UAER ) levels were divided into normal albuminuria group, microalbuminuria group and clinical proteinuria group , 35 health personnel were chosen as normal control group, the level of serum CysC concentration of four groups were and compared.Results Compared with the normal control group [( 0.55 ± 0.16)mg/L ] in normal albuminuria group, microalbuminuria group and clinical proteinuria group, the concentrations of CysC were signiifcantly increased ( 0.73 ± 0.18)mg/L, ( 1.21 ± 0.25)mg/L, (1.87 ± 0.24)mg/L (P<0.01 ), and the 3 groups of between CysC level has signiifcant difference (P<0.01), at the same time, microalbuminuria or clinical proteinuria group serum CysC concentrations and urinary protein excretion rate (UAER) showed signiifcant positive correlation ( r=0.083, 0.675,P<0.01 ).Conclusion Serum CysC is a relfection of renal function in patients with diabetic nephropathy ideal index, it has important signiifcance in the diagnosis of early diabetic nephropathy renal injury.
