目的探讨锁定加压钢板结合髂骨植骨治疗肱骨干下1/3骨不连的疗效。方法对19例肱骨干下1/3骨不连患者,取出原内固定,自体髂骨植骨,锁定加压钢板(LCP)固定,术后行X线片检查和肩、肘关节功能评估。结果本组19例均随访8个月~33个月,平均15.6个月,所有患者术后均骨折愈合,平均愈合时间4.9个月(3个月~8个月),术后并发桡神经麻痹2例。根据X线片及肩肘并节功能疗效判定:优13例,良4例,差2例,功能优良率为89.5%。结论肱骨干骨不连的最主要原因是固定装置不稳定,采用锁定加压钢板(LCP)结合髂骨植骨可提高固定强度,促进骨折愈合,能够获得较好的肩、肘功能。
目的探討鎖定加壓鋼闆結閤髂骨植骨治療肱骨榦下1/3骨不連的療效。方法對19例肱骨榦下1/3骨不連患者,取齣原內固定,自體髂骨植骨,鎖定加壓鋼闆(LCP)固定,術後行X線片檢查和肩、肘關節功能評估。結果本組19例均隨訪8箇月~33箇月,平均15.6箇月,所有患者術後均骨摺愈閤,平均愈閤時間4.9箇月(3箇月~8箇月),術後併髮橈神經痳痺2例。根據X線片及肩肘併節功能療效判定:優13例,良4例,差2例,功能優良率為89.5%。結論肱骨榦骨不連的最主要原因是固定裝置不穩定,採用鎖定加壓鋼闆(LCP)結閤髂骨植骨可提高固定彊度,促進骨摺愈閤,能夠穫得較好的肩、肘功能。
목적탐토쇄정가압강판결합가골식골치료굉골간하1/3골불련적료효。방법대19례굉골간하1/3골불련환자,취출원내고정,자체가골식골,쇄정가압강판(LCP)고정,술후행X선편검사화견、주관절공능평고。결과본조19례균수방8개월~33개월,평균15.6개월,소유환자술후균골절유합,평균유합시간4.9개월(3개월~8개월),술후병발뇨신경마비2례。근거X선편급견주병절공능료효판정:우13례,량4례,차2례,공능우량솔위89.5%。결론굉골간골불련적최주요원인시고정장치불은정,채용쇄정가압강판(LCP)결합가골식골가제고고정강도,촉진골절유합,능구획득교호적견、주공능。
Objective To investigate the clinical outcome of the locking compression plates and autologous iliac bone struct in the management of ununited of the distal third humerus shaft fracture. Methods 19 cases of the ununited distal third humerus shaft fracture were managed removal of original internal fixation ,internal fixation with locking compression plates(LCP)and autologous iliac bone struct. The outcomes were assessed by radiographs and evaluation of shoulder and elbow function. Results The mean duration of follow-up was 15.6 months (8~33months),all cases acquired bony union with a mean healing time of 4.9 months(3~8months), temporany palsy of the radial nerve occurred in 2 cases.The total excellent and fine rate of shoulder and elblow joint function was 89.5%.Conclusions Fixing device instability is the main reason for ununited of the humerus shaft fracture,the treatment of locking compression plates and autologous iliac bone struct can improve the fixed intensity enormously,improve bone healing and ensure good functional recovery of the upper extremity.
