目的:探讨冠状断面上动眼神经的解剖特征与MRI表现,为相关病变的影像诊断和外科手术提供解剖学依据.方法:取用成人尸头标本15例,经冰冻后,用电动断层带锯制成连续断层标本;用3.0 T MR扫描仪,获取10例成人志愿者的3D-CISS序列图像;在上述断层标本与MRI图像上观察动眼神经在冠状断面上的走行规律及毗邻关系.结果:动眼神经在经中脑脚间窝的断面上,呈“八”字形,紧贴两侧大脑脚向外下方走行,居大脑后动脉和小脑上动脉之间;在经海绵窦后份的断面上,位于“工”字形结构下端的左右两侧;在经海绵窦前份的断面上,位于前床突下方,颈内动脉的外侧;在经眶尖部的断面上,位于视神经和下直肌的外下方.结论:在冠状断面上可清晰显示动眼神经脑池段、海绵窦段和眶尖段的位置及其毗邻关系.

목적:탐토관상단면상동안신경적해부특정여MRI표현,위상관병변적영상진단화외과수술제공해부학의거.방법:취용성인시두표본15례,경빙동후,용전동단층대거제성련속단층표본;용3.0 T MR소묘의,획취10례성인지원자적3D-CISS서렬도상;재상술단층표본여MRI도상상관찰동안신경재관상단면상적주행규률급비린관계.결과:동안신경재경중뇌각간와적단면상,정“팔”자형,긴첩량측대뇌각향외하방주행,거대뇌후동맥화소뇌상동맥지간;재경해면두후빈적단면상,위우“공”자형결구하단적좌우량측;재경해면두전빈적단면상,위우전상돌하방,경내동맥적외측;재경광첨부적단면상,위우시신경화하직기적외하방.결론:재관상단면상가청석현시동안신경뇌지단、해면두단화광첨단적위치급기비린관계.
Objective:To describe the coronal anatomical features and MR imaging of the oculomotor nerve in order to provide anatomic data for the imaging diagnosis and surgery of oculomotor nerve lesions.Methods:15 sets of adult heads were frozen and sectioned with an electric bandsaw into serial coronal sections;10 healthy volunteers were examined by a 3D-CISS sequence using a 3.0-Tesla MR system; The sectional anatomy of the oculomotor nerve and its adjacent structures were identified on the coronal sectional plane and MR imaging.Results:The oculomotor nerve through the interpeduncal fossa runs lateral and inferiorly,shaping like the Chinese word "八",and passes between the posterior cerebral arteries and superior cerebellar arteries; The oculomotor nerve across the posterior part of cavernous sinus located on both of the left and right side of diaphragma sellae ; The oculomotor nerve across the anterior part of cavernous sinus located below the anterior clinoid process and outside the internal carotid artery ; The oculomotor nerve across the orbital apex located lateral inferior to the optic nerve and inferior rectus muscle.Conclusions:The coronal section can demonstrate accurately the distribution,location and the relations to surrounding tissues of oculomotor nerve.
