目的：观察腹腔镜与开腹胃癌根治术治疗进展期胃癌的围手术期疗效并进行对比。方法选取我院经腹腔镜及开腹手术治疗进展期胃癌患者61例，分为腔镜组和开腹组，比较两组患者围手术期疗效。结果腔镜组患者的手术时间较开腹组延长，但术后排气时间、住院时间均＜开腹组（P＞0.05），两组的术中出血、清扫淋巴结数量及术后近期并发症发生率无明显差异。结论腹腔镜胃癌根治术治疗进展期胃癌安全可行，无增加围手术期并发症，且具有创伤小、恢复快等优点，值得进一步研究和推广。
목적：관찰복강경여개복위암근치술치료진전기위암적위수술기료효병진행대비。방법선취아원경복강경급개복수술치료진전기위암환자61례，분위강경조화개복조，비교량조환자위수술기료효。결과강경조환자적수술시간교개복조연장，단술후배기시간、주원시간균＜개복조（P＞0.05），량조적술중출혈、청소림파결수량급술후근기병발증발생솔무명현차이。결론복강경위암근치술치료진전기위암안전가행，무증가위수술기병발증，차구유창상소、회복쾌등우점，치득진일보연구화추엄。
Objective To observe and compare laparoscopic and open radical gastrectomy in treatment of progressive gastric cancer during perioperative period. Method 61 patients with progressive gastric cancer and treated by laparoscopic or open radical gastrectomy were grouped and the clinical effects during perioperative period were compared. Result Though the surgery time of laparoscope group was longer than that of open group, the exhaust time and total hospital stay of laparoscope group were significantly decreased. There were no differences in Bleeding amount, lymph node dissected and complications during perioperative period between two groups. Conclusion Compared with traditional open radical gastrectomy, laparoscopic radical gastrectomy is safe and may be beneficial to patient’s fast recovery in the treatment of progressive gastric cancer.
