目的:探讨1.5 T磁共振SWI在慢性高血压患者检测脑内微出血的应用及临床价值。方法选择采用1.5 T磁共振SWI行慢性高血压脑内微出血( Cerebralmicrobleeds ,CMB )检查的患者,采用1.5 T超导型全身磁共振扫描仪,常规扫描及SWI扫描序列;确认并分析高血压引起的低信号CMB灶情况。结果全部患者诊断为高血压病,脑微出血的SWI表现信号特点为斑点或圆形低信号,分布于基底节区、丘脑区、皮层和皮层下、脑干、放射冠区等。其中伴有脑出血发作史有27例,脑梗死患者43例(含腔隙性脑梗死),排除脑血管畸形、脑肿瘤、脑外伤等。结论采用SWI检查显示慢性高血压脑内微出血症状,具有较高的敏感性与准确性,对早期预防脑出血及诊断脑卒中具有重要的指导作用。

목적:탐토1.5 T자공진SWI재만성고혈압환자검측뇌내미출혈적응용급림상개치。방법선택채용1.5 T자공진SWI행만성고혈압뇌내미출혈( Cerebralmicrobleeds ,CMB )검사적환자,채용1.5 T초도형전신자공진소묘의,상규소묘급SWI소묘서렬;학인병분석고혈압인기적저신호CMB조정황。결과전부환자진단위고혈압병,뇌미출혈적SWI표현신호특점위반점혹원형저신호,분포우기저절구、구뇌구、피층화피층하、뇌간、방사관구등。기중반유뇌출혈발작사유27례,뇌경사환자43례(함강극성뇌경사),배제뇌혈관기형、뇌종류、뇌외상등。결론채용SWI검사현시만성고혈압뇌내미출혈증상,구유교고적민감성여준학성,대조기예방뇌출혈급진단뇌졸중구유중요적지도작용。
Objective To study the application and clinical value of 1.5Tm agnetic resonance SW I in patients with chronic hypertension who were detected with cerebral microbleeds.Methods Sufferers undergoing the detection of chronic hypertension cerebral microbleeds by means of 1.5Tm agnetic resonance SW I were selected to accept routine scanning and SW I scanning sequence by 1.5Tm superconducting magnetic resonance im aging (MRI)scanner to identify and analyze lowsignal CMB focal condition caused by high blood pressure.Results All patients were diagnosed with hypertension characteristics,SW Im anifestation signals of cerebral microbleeds showed spot or circular lowsignals,distributed in basal ganglia region,lateral hypothalam icarea,cortical layer and subcortex,brainstem ,and corona radiata area. There were 27 cases of cerebral hem orrhage occurrence,43 cases of cerebral infarction (lacunar infarction included ),excluding cerebrovascular mal-form ation,cerebral tum or,and cerebral traum a.Conclusion SW I exam ination shows chronic hypertension cerebral microbleeds sym ptoms with high sensitivity and accuracy,and it plays an im portant guiding role in early prevention of cerebral hem orrhage and diagnosis of stroke.
