目的观察胸腺肽α1对老年慢性阻塞性肺病急性发作患者免疫功能和临床疗效的影响。方法选择120例老年慢性阻塞性肺病急性发作患者，随机等分为常规治疗组和联合治疗组，前者采取常规综合方法治疗，后者采取常规综合方法与胸腺肽α1联合治疗。在治疗前和治疗2w后检测记录患者免疫功能、临床有效率、感染复发率及住院天数。结果治疗后，两组IgG、IgA、IgM均无显著性变化（P ﹥0.05）；常规治疗组仅 CD4＋/CD8＋有显著增加（P ﹤0.05），而联合治疗组CD3＋、CD4＋、CD4＋/CD8＋及自然杀伤细胞表达均有显著增加（ P﹤0.05），CD8＋有显著减少（ P﹤0.05），且联合治疗组在CD3＋、CD4＋、CD8＋、CD4＋/CD8＋及自然杀伤细胞表达、临床有效率、感染复发率及住院天数等指标上，均优于常规治疗组（P﹤0.05）。结论常规的综合治疗联用胸腺肽α1能够显著改善老年慢性阻塞性肺病患者的细胞免疫功能，有效提高临床疗效，减少感染复发，值得临床推广应用。

목적관찰흉선태α1대노년만성조새성폐병급성발작환자면역공능화림상료효적영향。방법선택120례노년만성조새성폐병급성발작환자，수궤등분위상규치료조화연합치료조，전자채취상규종합방법치료，후자채취상규종합방법여흉선태α1연합치료。재치료전화치료2w후검측기록환자면역공능、림상유효솔、감염복발솔급주원천수。결과치료후，량조IgG、IgA、IgM균무현저성변화（P ﹥0.05）；상규치료조부 CD4＋/CD8＋유현저증가（P ﹤0.05），이연합치료조CD3＋、CD4＋、CD4＋/CD8＋급자연살상세포표체균유현저증가（ P﹤0.05），CD8＋유현저감소（ P﹤0.05），차연합치료조재CD3＋、CD4＋、CD8＋、CD4＋/CD8＋급자연살상세포표체、림상유효솔、감염복발솔급주원천수등지표상，균우우상규치료조（P﹤0.05）。결론상규적종합치료련용흉선태α1능구현저개선노년만성조새성폐병환자적세포면역공능，유효제고림상료효，감소감염복발，치득림상추엄응용。
Objective To observe the curative effect of thymosin α1 on senile patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD)and on their immune function. Methods 120 senile patients with AECOPD were randomly divided into 2 groups:routine treatment group and combined treatment group;routine treatment was performed in the 2 groups,while combined treatment with thymosin α1 was added to patients in combined treatment group;the immune function,the clinical effective rate,the reoccurrence and the days of hospitalization of the patients in both groups were detected before treatment and 2 weeks after treatment. Results After treatment,no obvious difference was found in IgG、IgA、IgM of the patients between the 2 groups(P﹥0. 05);the value of CD4 + /CD8 + of the patients in routine treatment group increased significant(P ﹤0. 05),while the values of CD3+, CD4 +,CD4 + /CD8 + and the natural killer cell expression increased significantly as well(P﹤0. 05)in combined treatment group,and the value of CD8 + decreased greatly(P﹤0. 05);the values of CD3+,CD4+,CD8+,CD4+ /CD8+,the natural killer cell expression, the clinical effective rate,the reoccurrence and the days of hospitalization of the patients in combined treatment group were all superior to those of the patients in routine treatment group(P﹤0. 05). Conclusions Thymosinα1 combined in routine treatment can obviously improve the immune function of the senile patients with AECOPD,raise the clinical effect and decrease the reoccurrence;it is worthy of wide clinical application.
