目的：了解医院门诊药房退药情况及原因，减少医院退药现象，以保证药品质量，促进合理用药。方法对我院门诊药房2013年9月~12月的退药情况进行统计，分析退药原因，探讨对策。结果四个月门诊药房共调配处方346460张，其中退药处方1654张，占0．48﹪。常见的退药原因主要包括药物不良反应、患者因素、医生因素及药房无药等几个方面。结论应建立严格的退药制度，规范退药程序，加强药师、医生和患者的沟通与协调，提高患者用药依从性，从而减少退药的发生。

Objective To know about medication withdrawals in outpatient dispensary and its reasons so as to guarantee the quality of medication and promote reasonable application of medication. Methods Data concerning medication withdrawals from September to December in 2013 in outpatient dispensary of our hospital were collected, through which the reasons were ana-lyzed and suggestions were put forward. Result A total of 346460 prescriptions were dispensed, of which withdrawal prescriptions were 1654, accounting for 0. 48%. The frequently encountered reasons for withdrawal included adverse reactions to the drug, pa-tients, doctors and short of stock. Conclusion The rules about medication withdrawal and procedure should be established and standardized. The communication and cooperation among pharmacists, doctors and patients should be strengthened to increase the compliance with medication of patients and reduce medication withdrawals.
