目的：总结原发于肺的黑色素瘤的临床特点。方法回顾性分析1例原发于肺的黑色素瘤患者的临床资料，并进行文献复习。结果60岁男性，因右侧胸痛、左下肢疼痛伴麻木为首发症状入院。行右肺上叶切除术+纵隔淋巴结清扫术。术后病理经免疫组化明确为肺恶性黑色素瘤。术后进行化疗及骨转移部位放疗，病情进展迅速，广泛转移。结论原发于肺的黑色素瘤十分罕见，症状无特异性，主要影像表现为肺孤立结节或团块，支气管内膜病变常见。HE染色结合免疫组化有助于明确诊断。部分患者外科手术切除可能获得长期生存。淋巴结受累往往提示预后不良。

Obsjective To summarize the clinical characteristics of primary pulmonary melanoma. Methods A case of primary pulmonary melanoma was reported, relevant cases and literature were reviewed. Results A 60-year-old man was admitted with the chief complaint of right chest pain, left leg pain and numbness. The patient un-derwent lobectomy of right upper lobe and resection of mediastinal lymph nodes. The pathology diagnosis of melanoma was supported by immunohistochemistry. After surgery, the patient received chemotherapy and radiotherapy of metastasis site. Then the condition of the patient deteriorated rapidly. The patient died of extensive metastases after 3 months of surgery. Conclusion Primary pulmonary melanoma is a very rare disease and the symptoms are nonspecific. CT scan always shows solitary nodule or mass in lung. Endobronchial disease is common. HE stain combined with immunohistochemistry is help-ful for diagnosis. Aggressive surgical resection offers possible long-term survival in some patients. The involvement of mediastinal lymph nodes indicates poor prognosis.
