目的:观察亚低温疗法对重症病毒性脑炎患儿血清和脑脊液中基质金属蛋白酶(MMPs)的干预作用。方法120例患儿随机分为实验组和对照组,每组60例。给予所有患儿抗病毒、对症等常规治疗。实验组患儿在常规治疗的基础上,加用5 d的选择性头部亚低温治疗。于治疗前、治疗中(第3天)和治疗后(第7天),测定和比较两组患儿血清和脑脊液中基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)的浓度。结果所有患儿血清和脑脊液中MMP-2和MMP-9的浓度都呈3 d后达到高峰、7 d明显下降的趋势。经过亚低温疗法的干预,实验组患儿血清和脑脊液中MMP-2和MMP-9的浓度在治疗中和治疗后均明显低于对照组患儿(P<0.01)。结论选择性头部亚低温疗法可明显抑制重症病毒性脑炎患儿血清和脑脊液中MMP-2和MMP-9的释放。

목적:관찰아저온요법대중증병독성뇌염환인혈청화뇌척액중기질금속단백매(MMPs)적간예작용。방법120례환인수궤분위실험조화대조조,매조60례。급여소유환인항병독、대증등상규치료。실험조환인재상규치료적기출상,가용5 d적선택성두부아저온치료。우치료전、치료중(제3천)화치료후(제7천),측정화비교량조환인혈청화뇌척액중기질금속단백매-2(MMP-2)화기질금속단백매-9(MMP-9)적농도。결과소유환인혈청화뇌척액중MMP-2화MMP-9적농도도정3 d후체도고봉、7 d명현하강적추세。경과아저온요법적간예,실험조환인혈청화뇌척액중MMP-2화MMP-9적농도재치료중화치료후균명현저우대조조환인(P<0.01)。결론선택성두부아저온요법가명현억제중증병독성뇌염환인혈청화뇌척액중MMP-2화MMP-9적석방。
Objective To observe the intervention of hypothermia therapy in the treatment of children with severe viral encephalitis on the levels of matrix metalloproteinases (MMPs) in the serum and cerebrospinal fluid.Methods A total of 120 cases with severe viral encephalitis were randomly divided into the experimental group and the control group, and each group contained 60 cases. All cases were treated with antiviral therapy and symptomatic treatment. The experimental group was treated with additional elective cephalic hypothermia treatment for 5 days. Levels of matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9) in the serum and cerebrospinal fluid before treatment, during treatment (the 3rd day) and after treatment (the 7th day) were detected and compared.Results The tendency of the levels of MMP-2 and MMP-9 in the serum and cerebrospinal fluid of all cases reached the peak at the 3rd day and decreased obviously at the 7th day. The levels of MMP-2 and MMP-9 in the serum and cerebrospinal fluid were significant lower in the experimental group than the control group during the treatment and after the treatment (P<0.01).Conclusion The selective cephalic hypothermia treatment can inhibit the release of the MMP-2 and MMP-9 in the serum and cerebrospinal fluid effectively.
