目的:应用脊髓血氧浓度相依对比(BOLD)功能成像技术,观察电针刺激曲池穴在颈脊髓磁共振功能成像激活情况。方法20例健康志愿者,电针刺激曲池穴,使用统计参数图(SPM2)软件得到健康志愿者脊髓内的激活区,观察激活区在矢状位和横断位上的功能成像激活特征。结果20例受试者颈脊髓功能成像均可以分析出脊髓神经功能激活区,横断面上激活区域主要位于同侧(右侧)脊髓后角,同侧前角及对侧后角也有不同程度激活;矢状面上功能激活区主要位于C4~7,高位颈脊髓C1~2也有少量激活。结论电针刺激右侧曲池穴均可以引起C1~7水平脊髓的功能激活,成像发现矢状位激活区主要位于C4~7,横断位激活区主要位于同侧(右侧)脊髓后角区,激活区域与脊髓神经反射通路基本一致。C1、C2、C3及延髓功能激活区可能是电针刺激曲池穴在颈脊髓的特定后效应功能区,可能提示针灸有复杂的脊髓内神经机制与经络独特的传感通道存在。

목적:응용척수혈양농도상의대비(BOLD)공능성상기술,관찰전침자격곡지혈재경척수자공진공능성상격활정황。방법20례건강지원자,전침자격곡지혈,사용통계삼수도(SPM2)연건득도건강지원자척수내적격활구,관찰격활구재시상위화횡단위상적공능성상격활특정。결과20례수시자경척수공능성상균가이분석출척수신경공능격활구,횡단면상격활구역주요위우동측(우측)척수후각,동측전각급대측후각야유불동정도격활;시상면상공능격활구주요위우C4~7,고위경척수C1~2야유소량격활。결론전침자격우측곡지혈균가이인기C1~7수평척수적공능격활,성상발현시상위격활구주요위우C4~7,횡단위격활구주요위우동측(우측)척수후각구,격활구역여척수신경반사통로기본일치。C1、C2、C3급연수공능격활구가능시전침자격곡지혈재경척수적특정후효응공능구,가능제시침구유복잡적척수내신경궤제여경락독특적전감통도존재。
ObjectiveTo observe the imaging activation condition of acusector stimulating Quchi point for cervical spinal cord through spinal cord blood oxygen level dependent (BOLD).MethodsThere were 20 healthy volunteers receiving acusector stimulating on Quchi point. Statistical parametric mapping 2 (SPM2) was used to get the active zone of volunteers’ spinal cord. Functional imaging activation was observed through sagittal view and transverse section view.ResultsFunctional magenetic resonance imaging could show the spinal cord nerve function active zone. The active zone on transverse section was mainly on the same side (right) of cornu posterius medullae spinalis, and there were also active zones in ipsilateral anterior angle and lientang angle. On the sagittal plane, active zone was mainly on C4~7, and high cervical spinal cord C1~2 also had a few active zone. ConclusionAcusector stimulating right Quchi point can cause functional activation of C2~7 spine. Imaging shows that active zones are mainly in C4~7 by sagittal view and in same side (right) of cornu posterius medullae spinalis by transverse section view. The active zones are nearly the same with spinal nerves reflex path. C1, C2, C3 and medulla oblongata functional activation zone may be the specific function zone after acusector stimulating Quchi point, and they can show that acupuncture and moxibustion have complicated sensing channel between spinal nerve and main and collateral channels.
