目的:观察无抽搐电休克治疗(MECT)中丙泊酚与硫喷妥钠对抑郁患者血压及认知功能的影响。方法将56例接受MECT治疗的抑郁症患者随机分为两组,在MECT时分别使用硫喷妥钠或丙泊酚诱导麻醉,疗程为8次,在治疗前和治疗结束后5 min记录患者心率(HR)、平均动脉压(MAP),治疗前和疗程结束1周后进行汉密顿抑郁量表(HAMD)、威斯康星卡片分类(WCST)、连续作业测验(CPT)、修订韦氏成人智力量表(WAIS-RC)测查评定。结果硫喷妥钠组MECT后MAP高于丙泊酚组( P<0.01),两组疗程结束1周后HAMD分均比治疗前降低( P<0.01),WCST,CPT,WAIS-RC评分比较无统计学差异( P>0.05)。结论丙泊酚用于MECT治疗麻醉诱导时,对患者认知功能无明显影响,同时对MAP影响小,可作为MECT首选麻醉药。

Objective To investigate the effects of thioPental sodium and ProPofol on BP and the cognitive function in the Patients with dePression during the modified electroconvulsive theraPy ( MECT ) . Methods 56 Patients with major dePression treated by MECT were randomly divided into 2 grouP and Performed the induced anesthesia with thioPental sodium and ProPofol resPectively during MECT. The course of treatment was 8 times. The heart rate ( HR ) and mean arterial Pressure ( MAP ) were recorded before theraPy and at 5 min af-ter MECT. DePression and the cognitive function were evaluated by using the Hamilton DePression Scale ( HAMD ) , the Wisconsin Card Sorting Test ( WCST ) , the Continuous Performance Test ( CPT ) and the Wechsler Adult Intelligence Scale-Revised China ( WAIS-RC ) before treatment and in 1 week after treatment. Results MAP in the thioPental sodium grouP was significanfly higher than that in the ProPofol grouP ( P<0. 01 ) . The HAMD scores in 1 week after theraPy in the two grouPs were significanfly decreased comPare with be-fore theraPy ( P<0. 01 ) . There was no statistical differences in the WCST, CPT, WAIS-RC scores ( P>0. 05 ) . Conclusion ProPofol has no obvious influence on the cognition function and little influence on MAP, and can serve as the first choice of anaesthetic for the in-duction of MECT.
