目的：探讨胸腺法新在肝脏移植患者中应用对急性排异反应发生率的影响及相关不良反应，评估其应用的安全性。方法选取2008年1月后超“杭州标准”的13例肝脏恶性肿瘤患者作为治疗组，肝移植术后1个月开始应用胸腺法新。应用方案为1．6 mg皮下注射，1次/d，应用2周后改为2次/周，连续应用2年。随机选取2008年1月前肝移植患者13例作为对照组，对照组未应用胸腺法新，免疫抑制方案等与治疗组相同，观察比较应用治过程中相关副作用及急性排异反应的发生情况。结果随访期间两组间肝功能恢复情况及排斥反应发生率均无明显统计学差异，胸腺法新治疗组和对照组各1例患者发生急性排异反应，治疗组急性排异反应患者经停用胸腺法新并调整免疫抑制剂后肝功能恢复正常；对照组即行排异反应患者经激素冲击治疗后得以纠正。两组比较急性排异反应发生率无显著差异（P＞0．05），治疗组未观察到其他该药物相关不良反应。结论肝脏移植患者应用胸腺法新不增加急性排异反应的发生率，在肝脏移植患者中应用是安全的。
목적：탐토흉선법신재간장이식환자중응용대급성배이반응발생솔적영향급상관불량반응，평고기응용적안전성。방법선취2008년1월후초“항주표준”적13례간장악성종류환자작위치료조，간이식술후1개월개시응용흉선법신。응용방안위1．6 mg피하주사，1차/d，응용2주후개위2차/주，련속응용2년。수궤선취2008년1월전간이식환자13례작위대조조，대조조미응용흉선법신，면역억제방안등여치료조상동，관찰비교응용치과정중상관부작용급급성배이반응적발생정황。결과수방기간량조간간공능회복정황급배척반응발생솔균무명현통계학차이，흉선법신치료조화대조조각1례환자발생급성배이반응，치료조급성배이반응환자경정용흉선법신병조정면역억제제후간공능회복정상；대조조즉행배이반응환자경격소충격치료후득이규정。량조비교급성배이반응발생솔무현저차이（P＞0．05），치료조미관찰도기타해약물상관불량반응。결론간장이식환자응용흉선법신불증가급성배이반응적발생솔，재간장이식환자중응용시안전적。
Objective Explore the security of the Thymalfasin in liver transplant patients.MethodsUse Thymalfasin in 13 liver transplant patients beyond HangZhou Criteria since Jan 2008.The medicine hadbeen given since the second month after liver transplantation.Once per day in the first 2 weeks and twice perweek in the following two years,1.6 mg Thymalfasin was injected hypodermically once.13 patients undergoneliver transplant before Jan 2008 were selected randomly as control group,who hadn't used Thymalfasin.Theimmunosuppression protocols were same in these two groups.The side reactions and the allograft rejection inthese two groups were observed.Results There was no statistical difference in liver function and allograftrejection incidence between the two groups.One candidate and one patient from control group were diagnosedacute allograft rejection in the follow-up period.The patient from Thymalfasin group was recovered bywithdrawing the Thymalfasin and adjusting the immunosuppressive agents.The patient frow control wasrecovered by Methyllprednisolone.The side effect were not observed in Thymalfasin group.Conclusions Thymalfasin couldn't increase the risk of allograft rejection and was safety in liver transplant patients.
