目的：探讨细胞角蛋白（CK ）19作为循环肿瘤细胞的间接标志物的价值，以及CK19在肾癌患者外周血中的表达水平以及临床意义。方法选取健康人50名作为对照组，50例肾细胞癌患者作为肾癌组，首先进行对照组和肾癌组间CK19的指标测定，采用流式细胞术检测上述人群外周血中CK19的表达情况；再分析肾癌组内C K 19的表达水平与肾癌病理分型、病理分期及组织学分级之间的关系。结果50名对照组中无一例CK19阳性，50例肾癌组中24例CK19阳性；不同肾癌病理分型之间CK19表达差异无统计学意义（ P＞0.05），不同肾癌病理分期及组织学分级之间CK19表达差异具有统计学意义（ P＜0．05）。结论 CK19能够初步判断患者外周血循环肿瘤细胞的水平，对早期无创性诊断肾癌、预测其生物学行为有一定的参考价值，值得进一步探索和研究。
목적：탐토세포각단백（CK ）19작위순배종류세포적간접표지물적개치，이급CK19재신암환자외주혈중적표체수평이급림상의의。방법선취건강인50명작위대조조，50례신세포암환자작위신암조，수선진행대조조화신암조간CK19적지표측정，채용류식세포술검측상술인군외주혈중CK19적표체정황；재분석신암조내C K 19적표체수평여신암병리분형、병리분기급조직학분급지간적관계。결과50명대조조중무일례CK19양성，50례신암조중24례CK19양성；불동신암병리분형지간CK19표체차이무통계학의의（ P＞0.05），불동신암병리분기급조직학분급지간CK19표체차이구유통계학의의（ P＜0．05）。결론 CK19능구초보판단환자외주혈순배종류세포적수평，대조기무창성진단신암、예측기생물학행위유일정적삼고개치，치득진일보탐색화연구。
Objective To detect the level of CK19 in peripheral blood in patients with renal cell carcinoma (RCC) and the healthy ,and to evaluate the clinical significance of CK 19 in the peripheral blood among RCC .Meth-ods Fifty healthies and 50 patients of RCC had been chosen .We used lymphocyte separation medium to enrich and then to detect CK19 by flow cytometry between control group and test group .The level of CK19 among test groups and relationship among CK19 and pathological classification ,pathological stage and histological grade were analyzed.Results Zero case of 50 in control group were positive .Twenty-four cases of 50 in test group were posi-tive .The difference of different pathological classification had no statistical meaning (P>0 .05) .The difference of different pathological stage and histological grade had statistical meaning (P<0 .05) .Conclusion By this experi-ment ,it is proved that the level of CK19 in peripheral blood can forecast the prognosis of RCC .To some extent the prognosis of RCC has correlate with CK19 theoretically ,but no statistical meaning is found between the two factors .This method is worthy to be studied.
