Three-dimensional transvaginal ultrasound%Uterine cavity lesion
目的:探讨经阴道三维超声(3D-TVS)和三维能量多普勒超声(3D-PDI)技术在宫腔病变诊断及鉴别诊断的临床应用价值。方法回顾性分析2011年5月~2012年12月在中山市博爱医院行阴道三维超声检查诊断宫腔病变,并进行诊断性刮宫或手术的60例患者,以病理学检查结果为金标准进行分析。结果60例患者中,病理诊断子宫内膜息肉37例,黏膜下型子宫肌瘤14例,子宫内膜增厚6例,子宫内膜癌3例,经阴道三维超声诊断与病理诊断一致为55例(91.7%),误诊5例(8.3%)。结论经阴道三维超声和三维能力多普勒技术可较好地显示病变的立体形态及与周边组织关系以及血供情况,可提高子宫腔内病变的诊断准确性,具有很高的临床应用价值。

Objective To explore three-dimensional transvaginal ultrasound (3D-TVS) and three-dimensional power Doppler imaging (3D-PDI) in the diagnosis and differential diagnosis of uterine cavity lesion.Methods A retrospective analysis of 60 cases in our hospital diagnostic uterine cavity lesion with 3D-TVS, and conduct diagnostic curettage or surgery,in pathological results as the gold standard for analysis.Results There were 37 cases were diagnosed as endometrial polyps, 14 cases as submucous myoma, 6 cases as endometrial hyperplasia,3 cases as endometrial cancer. All of which had specific characteristics. However, there were intracavitary abnormalities misdiagnosed or missed by three-dimensional transvaginal ultrasound (13.3%).Conclusion 3D-TVS through the three-dimensional reconstruction can better show the three-dimensional lesion morphology and the relationship with the surrounding tissue, can improve the accuracy of the uterine cavity lesion.
