目的:运用蛋白印迹法明确腭、肺及鼻咽上皮克隆(PLUNC)蛋白和TLR-4在鼻息肉和正常鼻黏膜组织之间的表达差异和相关性,进一步探讨二者在鼻息肉病因学及疾病进展中的作用。方法采用蛋白印迹法分别半定量检测45例鼻息肉和15名正常鼻黏膜组织中PLUNC蛋白和TLR-4的表达情况,并分析二者在鼻息肉组织中表达的相关性。结果①鼻息肉组与对照组鼻黏膜组织之间PLUNC蛋白和TLR-4表达差异均有统计学意义(P<0.05),鼻息肉组织中TLR-4高表达,PLUNC蛋白低表达;②鼻息肉组PLUNC蛋白和TLR-4表达呈负相关性(r=-0.384,P<0.05)。结论感染性因素在鼻息肉的发病过程中起着重要的作用,感染性因素的存在(特别是革兰阴性菌)使得鼻黏膜TLR-4表达量增高,鼻黏膜长期处于慢性炎症反应使得PLUNC表达分泌下降,可能促进了鼻息肉形成。

목적:운용단백인적법명학악、폐급비인상피극륭(PLUNC)단백화TLR-4재비식육화정상비점막조직지간적표체차이화상관성,진일보탐토이자재비식육병인학급질병진전중적작용。방법채용단백인적법분별반정량검측45례비식육화15명정상비점막조직중PLUNC단백화TLR-4적표체정황,병분석이자재비식육조직중표체적상관성。결과①비식육조여대조조비점막조직지간PLUNC단백화TLR-4표체차이균유통계학의의(P<0.05),비식육조직중TLR-4고표체,PLUNC단백저표체;②비식육조PLUNC단백화TLR-4표체정부상관성(r=-0.384,P<0.05)。결론감염성인소재비식육적발병과정중기착중요적작용,감염성인소적존재(특별시혁란음성균)사득비점막TLR-4표체량증고,비점막장기처우만성염증반응사득PLUNC표체분비하강,가능촉진료비식육형성。
Objective To investigate the difference in and correlation of PLUNC and toll-like receptor 4 (TLR-4) expression in nasal polyps tissue and normal nasal mucosa by immunoblotting techniques, and to determine the roles of these two markers in the pathogenesis of nasal polyps. Methods Forty-five specimens of nasal polyps and 15 of normal nasal mucosa were biopsied for Western blotting for semiquantitative assessment of PLUNC and TLR-4 ex-pression. The correlation between the two markers was also evaluated. Data were analyzed by using the SPSS 17.0 software. Results Compared with controls, the expression of TLR-4 was significantly increased and PLUNC reduced in nasal polyps (both P<0.05). There was negative correlation between PLUNC and TLR-4 in nasal polyps (r=-0.384, P<0.05). Conclusion Infective factors may play an important role in the pathogenesis of nasal polyps. The existence of infective factors (esp. Gram-negative bacteria) may increase TLR-4 expression in the nasal mucosa, which suppress PLUNC secretion leading to the development of nasal polyps.
