目的：对比支气管封堵导管与双腔支气管导管用于单肺通气的效果。方法选择40例需在单肺通气下行电视辅助胸腔镜手术（VATS）肺癌根治术患者，随机分为 A 组（行支气管封堵）和 B 组（插双腔管），每组20例。观察并记录两组的插管时间、定位时间、单肺通气时气道压的变化、术中肺萎陷的质量以及术后声带和气道损伤的比较。结果 A 组的插管时间、定位时间明显短于 B 组（P <0.05），术后声带和气道损伤的发生率 A 组明显低于B 组（P <0.05）；单肺通气时 A 组气道压明显低于 B 组（P <0.05）。结论支气管封堵导管与双腔支气管导管用于单肺通气均安全有效，但支气管封堵导管行单肺通气操作更简单快捷，单肺通气期间气道压低，术后不良反应少。
목적：대비지기관봉도도관여쌍강지기관도관용우단폐통기적효과。방법선택40례수재단폐통기하행전시보조흉강경수술（VATS）폐암근치술환자，수궤분위 A 조（행지기관봉도）화 B 조（삽쌍강관），매조20례。관찰병기록량조적삽관시간、정위시간、단폐통기시기도압적변화、술중폐위함적질량이급술후성대화기도손상적비교。결과 A 조적삽관시간、정위시간명현단우 B 조（P <0.05），술후성대화기도손상적발생솔 A 조명현저우B 조（P <0.05）；단폐통기시 A 조기도압명현저우 B 조（P <0.05）。결론지기관봉도도관여쌍강지기관도관용우단폐통기균안전유효，단지기관봉도도관행단폐통기조작경간단쾌첩，단폐통기기간기도압저，술후불량반응소。
Objective To compare the effects of the bronchial blockers(BBs)tube and double-lumen endotracheal tube(DLT)in one-lung venti lation. Methods Forty patients scheduled for videoas-sisted thoracoscopy(VAB)under one-lung ventilation for radical reseation of pulmonary carcinoma were chose,and were randomly divided into group A(bronchial blockers tube)and grouup B(DLT),with 20 cases in each group. The following variables were recorded:intubate the assigned tube time,position the assigned tube,time. The pulmonary collapse extent,arway pressure during one-lung ventilation,Postop-erative incidence of the airway damage. Results The intubation time,position the assigned tube time, and the incidence of postoperative airway damage in the group A were significantly lower than those in the group B(P < 0. 05). Airway pressure during one-lung ventilation in the group A was significantly lower than that in the group B(P < 0. 05). Conclusions The bronchial blockers tube is more convenient than double-lumen endotracheal tube in one-lung ventilation,and the airway pressure is low,postoperative ad-werse reaction is less.
