目的:观察重组人促红细胞生成素(EPO)与铁剂联合治疗早产儿贫血的效果。方法选取2012年1月至2013年11月东华医院新生儿科收治的早产儿贫血患儿52例,治疗组28例,对照组24倒。治疗组给予人促红细胞生成素与铁剂联合连续治疗3周,对照组按常规方法治疗。对两组患儿治疗1周及3周后的结果进行统计学分析。结果治疗组组内比较,治疗7 d、21 d 后 EPO、网织红细胞(Ret)水平较治疗前增加,差异有统计学意义(P <0.01);治疗21 d 后红细胞计数(RBC)、血细胞比容(HCT)、血红蛋白(Hb)水平较治疗前增加,差异有统计学意义(P <0.05)。治疗组与对照组组间比较,治疗21 d 后 RBC、HCT、Hb、EPO、Ret 水平差异有统计学意义(P <0.05)。结论应用重组人促红细胞生成素与铁剂联合早期干预治疗新生儿贫血的临床效果较好,能有效减少输血或避免输血,且用药安全,接近生理途径,值得推广。

목적:관찰중조인촉홍세포생성소(EPO)여철제연합치료조산인빈혈적효과。방법선취2012년1월지2013년11월동화의원신생인과수치적조산인빈혈환인52례,치료조28례,대조조24도。치료조급여인촉홍세포생성소여철제연합련속치료3주,대조조안상규방법치료。대량조환인치료1주급3주후적결과진행통계학분석。결과치료조조내비교,치료7 d、21 d 후 EPO、망직홍세포(Ret)수평교치료전증가,차이유통계학의의(P <0.01);치료21 d 후홍세포계수(RBC)、혈세포비용(HCT)、혈홍단백(Hb)수평교치료전증가,차이유통계학의의(P <0.05)。치료조여대조조조간비교,치료21 d 후 RBC、HCT、Hb、EPO、Ret 수평차이유통계학의의(P <0.05)。결론응용중조인촉홍세포생성소여철제연합조기간예치료신생인빈혈적림상효과교호,능유효감소수혈혹피면수혈,차용약안전,접근생리도경,치득추엄。
Objective To observe the effects of recombinant human erythropoietin combined with iron on anemia in premature newborn. Methods From January 2012 to November 2013,52 cases of anemia in premature newborn in our hospital were chose,including 28 patients in the treatment group and 24 patients in the control group. Patients in the treatment group received recombinant human erythro-poietin combined with iron for 3 weeks,and the patients in the control group received conventional meth-ods. The results of the two groups after 1 week and 3 weeks of treatment were statistically analyzed. Re-sults In treatment group,after 7 days and 21 days of treatment,the levels of EPO and reticulocyte were higher than those before treatment,the differences were significant(P <0. 01);after 21 days of treatment, the levels of RBC,HCT,Hb were higher than those before treatment,the differences were significant(P <0. 05). There were significant differences in the levels of RBC,HCT,Hb,EPO,Ret after 21 days of treat-ment between the two groups(P <0. 05). Conclusions The clinical effect of recombinant human erythro-poietin combined with iron on early intervention of neonatal anemia is good and safe,it can effectively re-duce or avoid blood transfusions,and is close to the physiological pathways,so is worthy of promotion.
