目的：探讨彩超引导下经皮腹腔穿刺置管冲洗引流治疗急性重症胰腺炎的临床价值。方法52例急性重症胰腺炎患者，穿刺组31例均在内科药物保守治疗基础上，早期彩超引导下行经皮腹腔穿刺置管外引流冲洗术；对照组21例仅行内科保守治疗。观察指标为腹腔感染率、胰腺出血率、胰腺脓肿率、中转手术率、住院时间及住院费用。结果穿刺组在腹腔感染率、胰腺出血率、胰腺脓肿率、中转手术率、住院时间及住院费用方面与对照组比较差异均有统计学意义（P 均<0.05）。结论合理使用彩超引导经皮穿刺置管引流可以降低外科开放手术及其并发症发生率，并减少住院时间及费用。

Objective To investigate the clinical value of ultrasound guided percutaneous punc-ture catheter drainage on acute severe pancreatitis. Methods Among the 52 patients with severe acute pancreatitis,31 cases in puncture group were given the early color ultrasound guided percutaneous punc-ture drainage and irrigation based on the internal medicine conservative treatment,21 cases in control group only received internal medicine conservative treatment. The abdominal infection rate,pancreatic bleeding rate,pancreatic abscess rate,transfer to operation rate,operation time and expenditure in hos-pital were observed between the two groups. Results Compared with the control group,the abdominal infection rate,pancreatic bleeding rate,pancreatic abscess rate,transfer to operation rate,hospitalization time and hospital costs in puncture group had significant differences(P all < 0. 05). Conclusions The ra-tional use of ultrasound guided percutaneous catheter drainage can reduce the incidences of open opera-tion and surgery complications and reduce the hospitalization time and costs.
