目的：探讨血清癌胚抗原（CEA）、癌抗原19-9（CA19-9）、癌抗原125（CA125）、癌抗原724（CA724）、癌抗原50（CA50）对胰腺癌的临床诊断价值。方法用雅培 ARCHITECT i2000型化学发光仪检测18例胰腺癌患者、20例胰腺术后患者、15例胰腺炎患者及14例健康体检者血清 CEA、CA19-9、CA125、CA724、CA50的含量，胰腺癌组分别与良性病变组和正常对照组比较。结果胰腺癌组中 CA19-9、CA125、CEA 血清水平较胰腺良性疾病组和健康查体组明显升高，其中以血清 CA19-9水平升高最显著。与健康人相比，胰腺良性病组 CA19-9、CEA、Ca724升高，差异有统计学意义（ P ﹤0.05）。而各组 CA724未出现统计学差异。CA19-9诊断胰腺癌灵敏度为77.8%，特异度为83.7%，准确率为79.1%。CA19-9＋ CA125联合检测灵敏度和特异度最高，分别为100%和79.6%，准确率为85.1%。结论血清CA19-9、CEA、CA125在胰腺癌的诊断中有一定的临床应用价值，联合应用可提高胰腺癌的诊断率。
목적：탐토혈청암배항원（CEA）、암항원19-9（CA19-9）、암항원125（CA125）、암항원724（CA724）、암항원50（CA50）대이선암적림상진단개치。방법용아배 ARCHITECT i2000형화학발광의검측18례이선암환자、20례이선술후환자、15례이선염환자급14례건강체검자혈청 CEA、CA19-9、CA125、CA724、CA50적함량，이선암조분별여량성병변조화정상대조조비교。결과이선암조중 CA19-9、CA125、CEA 혈청수평교이선량성질병조화건강사체조명현승고，기중이혈청 CA19-9수평승고최현저。여건강인상비，이선량성병조 CA19-9、CEA、Ca724승고，차이유통계학의의（ P ﹤0.05）。이각조 CA724미출현통계학차이。CA19-9진단이선암령민도위77.8%，특이도위83.7%，준학솔위79.1%。CA19-9＋ CA125연합검측령민도화특이도최고，분별위100%화79.6%，준학솔위85.1%。결론혈청CA19-9、CEA、CA125재이선암적진단중유일정적림상응용개치，연합응용가제고이선암적진단솔。
Objective To explore the clinical diagnostic significance of serum CA19 - 9,CEA,CA125,CA724 and CA50 in patients with malignant tumors of digestive system. Methods Serum levels of CA19 - 9,CEA,CA125,CA724 and CA50 were detected by Abbott AR-CHITECT i2000 from 18 patients with pancreatic cancer,35 patients with pancreatic benign diseases(20 patients with post - pancreatic operation and 15 patients with acute pancreatitis)and 14 healthy controls. Results Serum levels of CA19 - 9,CA125 and CEA were significantly higher in pancreatic cancer group than those of benign pancreatic disease group,while serum level of CA19 - 9 was the highest(646. 33 ± 124. 15 ug/ ml). In comparison with healthy controls,serum levels of CA19 - 9,CEA and CA724 were increased in patients with benign pancreatic diseases,and the difference was statistically significant( P ﹤ 0. 05). There was no difference in serum level of CA724 among these three groups. The sensitivity of CA19 - 9 was 77. 8% ,its specificity was 83. 7% and accuracy was 79. 1% . The sensitivity and specificity of combined detection of CA19 - 9 and CA125 were 100% and 79. 6% respectively,and its rate of accuracy was 85. 1% . Conclusion Serum levels of CAl99,CEA and CA125 have clinical significance in diagnosis of patients with pancreatic cancer,and the combined detection of tumor markers can raise the diagnostic rate of pancreatic cancer.
