目的 比较三种检测方法在诊断女性淋病的应用价值.方法 取宫颈分泌物进行淋病奈瑟氏菌培养、直接涂片和快速试纸法检测,并以培养法为标准进行比较分析.结果 326例可疑的女性淋病患者中,培养法阳性率为19.3%,直接涂片法阳性率为14.4%,快速试纸法阳性率为22.1%;直接涂片法和快速试纸法灵敏度分别为57.1%、76.2%,特异度分别为95.8%、90.9%,Youden指数分别为0.53、0.67;革兰氏阴性双球菌和阳性球菌均可造成直接涂片法的假阳性,莫拉菌和其它奈瑟菌等也可与快速试纸产生阳性反应.结论 淋病快速试纸法在诊断女性淋病上较直接涂片法具有更好的实用价值,但在诊治中无法替代培养法的地位.

Objective To compare the diagnostic value of the three methods for female gonorrhea.Methods Cervical secretions were examined using Neisseria gonorrhoeae culture,direct smear,and fast test paper.the result of Neisseria gonorrhoeae culture were standardized.Results Among the 326 female gonorrhea specimens,the positive rates of Neisseria gonorrhoeae culture,direct smear,and fast test paper were 19.3%,14.4%,and 22.1%,respectively.The sensitivitis were 57.1% and 76.2%,the specificities were 95.8% and 90.9%,and the Youden indexes were 0.53 and 0.67 for direct smear and fast test paper,respectively.Gram-negative diplococcic and positive cocci both can lead to false positive in direct smear.Moraxella and other Neisseria can react positively to fast test paper.Conclusions Fast test paper is more valuable than direct smear in the diagnosis of female gonorrhea,but can't replace the position of Neisseria gonorrhoeae culture.
