目的探讨泪道栓子治疗视频终端顽固性干眼症的临床效果.方法临床病例对照研究.选择2011年5～10月在沈阳爱尔眼视光医院明确诊断为视频终端顽固性干眼症的患者65例(130只眼),按就诊时间顺序随机分为试验1组31例(62只眼)和试验2组34例(68只眼).试验l组为上、下泪小点植入永久性泪道栓,试验2组为上泪小点植入临时性自溶性泪道栓(3个月),下泪小点植入永久性泪道栓子.治疗后3个月、6个月及1年进行复诊,观察治疗前及治疗后3个月、6个月、1年患者自觉症状、SIT试验、泪膜破裂时间(BUT)、角膜荧光染色(FL)评分的改变结果.结果患者性别、年龄无明显差异.试验1组与试验2组治疗后3个月、6个月、1年患者自觉症状、SIT试验、泪膜破裂时间(BUT)、角膜荧光染色(FL)评分的改变较治疗前均有好转改变.试验1组与试验2组在治疗后3个月、6个月及1年在患者、SIT试验、泪膜破裂时间(BUT)、角膜荧光染色(FL)评分方面差异无统计学意义(t值分别为0.629,-1.306,0.452,P值为0.532,0.196,0.653,P＞0.05).结论使用泪道栓子进行泪道栓塞术可以明显改善干眼患者的临床症状,增加患者眼球表面泪液量,改善泪膜的稳定性,是治疗视频终端顽固性于眼的有效方法之一;上、下泪小点植入永久性泪道栓及上泪小点植入临时性自溶性泪道栓(3个月),下泪小点植入永久性泪道栓子对治疗视频终端顽固性干眼症差异无统计学意义.

Objective To explore the clinical effects of lacrimal emboli in the treatment of refractory dry eye caused by video terminal syndrome.Methods Sixty-five patients (130 eyes) from May 2011 to October 2011,Shenyang Aier Eye Hospital,diagnosed as refractory dry eye due to the video terminal were randomly divided into an experimental group one contained 31 patients (62 eyes) and group two 34 patients (68 eyes).Group one received a upper and lower tear duct,the group two received temporary plug on the upper while permanent plug implantation on the lower lacrimal tear duct.Patients symptoms,SIT testing,tear film breakup time (BUT),corneal fluorescein staining (FL) score change results were observed on pre-treatment,3 months,6 months and 1 year after treatment.Results There was no significant differenced between gender and age.After treatment patients symptoms,SIT test,tear film breakup time (BUT),corneal fluorescein staining (FL)rate of changes were significantly improved compared with before treatment.There was no significant difference (P ＞0.05) between groups and observing time.Conclusions Use the tear duct plugs embolization can significantly improve the clinical symptoms of dry eye,increase tear volume in patients with ocular surface and improve the stability of the tear film,especially on the treatment of refractory dry due to video terminal.Both the permanent and temporary lacrimal plug implantation is effective method on treatment of dry eye and there is no significant difference between them.
