目的:研究 MDM2在胃癌组织中的表达意义及其与幽门螺杆菌 L型(Helicobacter pylori-L,Hp-L)感染在胃癌发生中的关系。方法(1)应用革兰染色法和免疫组化学法检测100例胃癌及对应的40例癌旁正常胃黏膜组织(对照组)中Hp-L型感染和 MDM2蛋白的表达情况;(2)应用逆转录多聚酶链反应(RT-PCR)方法检测 MDM2的 mRNA在30例新鲜胃癌组织及其对应的癌旁正常胃粘膜组织(对照组)中的表达情况。结果100例胃癌组织及对应的40例癌旁正常胃粘膜组织中 MDM2阳性表达率为71.0%(71/100)和42.5%(17/40),可见 MDM2在胃癌组织中高表达和在非胃癌组中低表达具有统计学意义(P<0.05),且 MDM2表达水平升高与肿瘤大小、浸润深度、淋巴结转移和TNM分期有关(P<0.05),与性别、年龄无关(P>0.05);RT-PCR显示,肿瘤组织 MDM2的表达明显高于远端正常胃黏膜对照组织的表达量且差异明显(P<0.01)。革兰染色和免疫组化两种方法检测 Hp-L型结果具有一致性(P>0.05),胃癌组织中 Hp-L型感染阳性组的 MDM2表达阳性率高于 Hp-L型阴性组(P<0.05),且 Hp-L型阳性率和MDM2蛋白表达呈正相关(r=0.447,P<0.05)。结论 MDM2蛋白在胃癌中呈高表达,且与胃癌的浸润、转移有关,其机制可能与 Hp-L型感染有关。

목적:연구 MDM2재위암조직중적표체의의급기여유문라간균 L형(Helicobacter pylori-L,Hp-L)감염재위암발생중적관계。방법(1)응용혁란염색법화면역조화학법검측100례위암급대응적40례암방정상위점막조직(대조조)중Hp-L형감염화 MDM2단백적표체정황;(2)응용역전록다취매련반응(RT-PCR)방법검측 MDM2적 mRNA재30례신선위암조직급기대응적암방정상위점막조직(대조조)중적표체정황。결과100례위암조직급대응적40례암방정상위점막조직중 MDM2양성표체솔위71.0%(71/100)화42.5%(17/40),가견 MDM2재위암조직중고표체화재비위암조중저표체구유통계학의의(P<0.05),차 MDM2표체수평승고여종류대소、침윤심도、림파결전이화TNM분기유관(P<0.05),여성별、년령무관(P>0.05);RT-PCR현시,종류조직 MDM2적표체명현고우원단정상위점막대조조직적표체량차차이명현(P<0.01)。혁란염색화면역조화량충방법검측 Hp-L형결과구유일치성(P>0.05),위암조직중 Hp-L형감염양성조적 MDM2표체양성솔고우 Hp-L형음성조(P<0.05),차 Hp-L형양성솔화MDM2단백표체정정상관(r=0.447,P<0.05)。결론 MDM2단백재위암중정고표체,차여위암적침윤、전이유관,기궤제가능여 Hp-L형감염유관。
Obj ective To investigate the expression of the MDM2 protein in gastric carcinoma and its relationship with the Helicobacter pylori L-form (Hp-L)infection.Methods (1)Immunohistochemical and Gram-staining were used to detect the Hp-L infection and the expression of MDM2 protein in 100 samples of gastric cancer tissue and 40 samples of the corresponding adjacent tissues (control group).(2)The ex-pression of MDM2 mRNA in 30 cases of fresh gastric cancer tissue and the correspording normal gastric mucosa (control group)were detected by semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR).Results The rate of positive expressions of MDM2 gene in gastric cancers and adj acent tissue were 71.0%(71/100)and 42.5%(17/40),the positive rate of MDM2 protein expression in gastric carcino-ma was significantly higher than in the control group (P<0.05),MDM2 protein expression increased with tumor size,depth of invasion,lymph node metastasis and TNM stage (P <0.05),not correlated with pa-tients'sex and age (P>0.05 ).RT-PCR showed significant differences MDM2 expression in between tumor tissue and distal normal control tissues(P<0.01).The results by Gramstaining and immunohis to-chemistry for Hp-L type were consistent (P>0.05).In gastric cancer,group ,the expression of MDM2 was significantly higher in the Hp-L positive group than that in negative group (P<0.05).There was a significant positive relationship of Hp-L with MDM2 (r=0.447,P<<0.05).Conclusion The high ex-pression of MDM2 protein and mRNA in gastric cancer was increased and correlated with tumor invasion and metastasis,and the mechanism may be related to Helicobacter pylori L-form infection.
