目的:评估吸入性伊洛前列素治疗全腔肺动脉连接术( total cavopulmonary connection, TCPC)后肺动脉高压( pulmongary arterial hypertension, PAH)临床疗效。方法选择2011年1月-2013年12月行 TCPC术后合并PAH患儿12例,在停体外循环( cardiopulmonary bypass, CPB)后超声雾化吸入伊洛前列素,观察吸入前、吸入结束、吸入结束后30 min血流动力学、呼吸及血气分析参数。结果肺动脉收缩压、肺动脉压/体动脉压及中心静脉压吸入结束、吸入结束后30 min均低于吸入前,氧和指数高于吸入前( P<0.05)。结论 TCPC术后吸入伊洛前列素可选择性舒张肺血管,降低肺循环阻力,改善肺部血流动力学。

목적:평고흡입성이락전렬소치료전강폐동맥련접술( total cavopulmonary connection, TCPC)후폐동맥고압( pulmongary arterial hypertension, PAH)림상료효。방법선택2011년1월-2013년12월행 TCPC술후합병PAH환인12례,재정체외순배( cardiopulmonary bypass, CPB)후초성무화흡입이락전렬소,관찰흡입전、흡입결속、흡입결속후30 min혈류동역학、호흡급혈기분석삼수。결과폐동맥수축압、폐동맥압/체동맥압급중심정맥압흡입결속、흡입결속후30 min균저우흡입전,양화지수고우흡입전( P<0.05)。결론 TCPC술후흡입이락전렬소가선택성서장폐혈관,강저폐순배조력,개선폐부혈류동역학。
Objective To evaluate the efficacy of inhaled Iloprost in treatment of patients with pulmonary arterial hypertension ( PAH) after total cavopulmonary connection ( TCPC) . Methods Twelve patients with PAH after TCPC during January 2011 and December 2013 underwent Iloprost by ultrasonic atomizing inhalation after cardiopulmonary by-pass ( CPB) was stopped. The indexes of hemodynamics, respiration and blood gas analysis were observed before inhala-tion, at the end of the inhalation and 30 min after inhalation cessation. Results The values of pulmonary arterial systolic pressure, pulmonary arterial pressure/ systemic artery pressure ( Pp/Ps) and central venous pressure ( CVP) were signif-icantly decreased, while the oxygen index ( OI) was significantly increased at the end of the inhalation and 30 min after inhalation cessation, compared with those before inhalation ( P<0. 05 ) . Conclusion Inhaled Ilopost is an effective treatment for selective reduction of pulmonary vascular resistance for patients after TCPC, and can improve the pulmonary blood flow.
