目的：探讨辨证施护干预联合活血通络凝胶以及镜下清理术在骨性关节炎中的应用效果。方法选取2008年7月至2010年2月我院收治的KOA患者96例，随机分为干预组和对照组。对照组患者采用镜下关节清理术联合扶他林乳膏外敷疗法，术后给予常规护理；干预组患者采用镜下关节清理术辅以活血通络凝胶进行外敷治疗，术后采取中医辨证施护。疗程结束后观察两组临床疗效及其对血液与关节液中IL-1水平，并评价护理服务满意率。结果干预组患者治疗总有效率明显高于对照组（P＜0.05），患者血清与关节液中IL-1水平均明显低于对照组（均P＜0.05），患者护理服务满意率明显高于对照组（P＜0.05）。结论在膝关节骨性关节治疗中采用辨证施护干预辅以活血通络凝胶外敷及镜下关节清理术能明显减低KOA病患血液及关节液中IL-1水平，对患者减轻疼痛及尽快恢复关节活动功能有利，临床疗效满意。
목적：탐토변증시호간예연합활혈통락응효이급경하청리술재골성관절염중적응용효과。방법선취2008년7월지2010년2월아원수치적KOA환자96례，수궤분위간예조화대조조。대조조환자채용경하관절청리술연합부타림유고외부요법，술후급여상규호리；간예조환자채용경하관절청리술보이활혈통락응효진행외부치료，술후채취중의변증시호。료정결속후관찰량조림상료효급기대혈액여관절액중IL-1수평，병평개호리복무만의솔。결과간예조환자치료총유효솔명현고우대조조（P＜0.05），환자혈청여관절액중IL-1수평균명현저우대조조（균P＜0.05），환자호리복무만의솔명현고우대조조（P＜0.05）。결론재슬관절골성관절치료중채용변증시호간예보이활혈통락응효외부급경하관절청리술능명현감저KOA병환혈액급관절액중IL-1수평，대환자감경동통급진쾌회복관절활동공능유리，림상료효만의。
Objective To investigate the effect of syndrome nursing with blood circulation network gel external application and ar -throscopic debridement in knee osteoarthritis and the influence on IL -1.Methods A total of 96 cases of knee osteoarthritis patients were randomly divided into intervention group (47cases) and control group (49 cases) , intervention group were treated with arthroscopic debride-ment and blood circulation network gel external application , syndrome nursing was given after operation , control group was treated with ar-throscopic debridement and voltaren cream external application , conventional care was given after operation .Clinical effect and the influence on IL-1 were observed , and the nursing satisfaction rate was assessed .Results The total treatment efficiency and nursing satisfaction rate in intervention group were better than that in control group (P<0.05), there was significant difference , and IL-1in serum and synovial fluid was less than that in control group .Conclusions In the treatment of KOA , Syndrome nursing with blood circulation network gel exter-nal application and arthroscopic debridement can reduce IL -1 in serum and synovial fluid , and is good for joint function recovery , clinical results is satisfied.
