目的：总结微切口、扭动超声乳化技术在不同核级白内障手术中的应用特点和临床效果。方法：2010年3月～2012年12月，1500眼老年性白内障采用微切口（2.2mm）、扭动超声乳化白内障吸除联合人工晶体植入术治疗，对比不同核级白内障超声时间(U/S total time,U/STT)、累计释放能量(cumulative dissipated energy，CDE)、总液体消耗量(total fluid used, TFU)，并统计无眼部伴发疾病的单纯白内障术后早期平均裸眼视力。结果：硬核组U/STT、CDE、TFU明显大于软核组和中等硬核组，p<0.01，软核组与中等硬核组视力好于硬核组，p<0.05。无眼部伴发疾病的单纯白内障（1090眼）术后3天裸眼视力≥0.5者占89%，≥1.0者25%。结论：微切口、扭动超声乳化技术安全可靠、治疗效果良好，但应掌握不同核级白内障超声乳化技术的运用特点。

목적：총결미절구、뉴동초성유화기술재불동핵급백내장수술중적응용특점화림상효과。방법：2010년3월～2012년12월，1500안노년성백내장채용미절구（2.2mm）、뉴동초성유화백내장흡제연합인공정체식입술치료，대비불동핵급백내장초성시간(U/S total time,U/STT)、루계석방능량(cumulative dissipated energy，CDE)、총액체소모량(total fluid used, TFU)，병통계무안부반발질병적단순백내장술후조기평균라안시력。결과：경핵조U/STT、CDE、TFU명현대우연핵조화중등경핵조，p<0.01，연핵조여중등경핵조시력호우경핵조，p<0.05。무안부반발질병적단순백내장（1090안）술후3천라안시력≥0.5자점89%，≥1.0자25%。결론：미절구、뉴동초성유화기술안전가고、치료효과량호，단응장악불동핵급백내장초성유화기술적운용특점。
Objective To summarize the clinical effect of micro-incision,co-axial twist phacoemulsification and analyze its application features in different grade nucleus cataract.Methods in our hospital from March 2010 to December 2012,a total of 1500 eyes of cataract underwent micro-incision,co-axial twist phacoemulsification and intraocular lens implantation surgery by one experienced surgeon,U/S total time(U/STT), cumulative dissipated energy(CDE) and total fluid used(TFU) was recorded and compared between cataract with different grade nucleus, soft nucleus(SN)group,moderate hard nucleus(MHN)group and hard nucleus(HN)group, the early uncorrected visual acuity in the pure cataract which without other eye disease in 1090 eyes, was checked and compared.Results U/STT ), CDE and TFU in HN group was larger than those in SN and MHN group, p<0.01;89% of the eyes with pure cataract achieved the uncorrected visual acuity to 0.5 or more at 3 days postoperatively, 50.5% of the eyes to 0.8 or more, 25% of the eyes to 1.0 or more,and the uncorrected visual acuity in the group SN and MHN was much more better than that in the group HN in the early days. Conclusions The micro-incision, twist phacoemulsification technique has the advantages of safety and efficacy,and its application feature in different grade nucleus cataract must be known wel .
