目的:探讨肺表面活性物质(pulmonary surfactant,PS)治疗新生儿胎粪吸入综合征(meconium aspiration syn-drome,MAS)的临床效果。方法选择MAS患儿84例,随机分为两组,各42例。对照组予以机械通气和常规治疗,观察组在此基础上加用PS治疗,对比两组临床治疗效果。结果观察组患儿治愈率为95.24%,高于对照组的80.95%( P <0.05);观察组住院时间为(12.09±3.11)d,短于对照组的(17.81±4.07)d ( P <0.01)。两组治疗前动脉二氧化碳分压(PaCO2)和动脉氧分压(PaO2)差异均无统计学意义( P >0.05);治疗24 h后,观察组PaCO2低于对照组( P <0.01),PaO2高于对照组( P<0.01)。结论机械通气联合PS治疗新生儿MAS临床效果明显,有利于改善血气情况。

목적:탐토폐표면활성물질(pulmonary surfactant,PS)치료신생인태분흡입종합정(meconium aspiration syn-drome,MAS)적림상효과。방법선택MAS환인84례,수궤분위량조,각42례。대조조여이궤계통기화상규치료,관찰조재차기출상가용PS치료,대비량조림상치료효과。결과관찰조환인치유솔위95.24%,고우대조조적80.95%( P <0.05);관찰조주원시간위(12.09±3.11)d,단우대조조적(17.81±4.07)d ( P <0.01)。량조치료전동맥이양화탄분압(PaCO2)화동맥양분압(PaO2)차이균무통계학의의( P >0.05);치료24 h후,관찰조PaCO2저우대조조( P <0.01),PaO2고우대조조( P<0.01)。결론궤계통기연합PS치료신생인MAS림상효과명현,유리우개선혈기정황。
Objective To explore the clinical effect of pulmonary surfactant ( PS) on the treatment of meconium aspiration syn-drome(MAS).Methods Eighty-four children with MAS were randomly divided into two groups ( n =42,each).The control group was treated with mechanical ventilation and conventional therapy ,based on which ,the observation group was given additional pulmonary surfactant.Then,therapeutic effect of the two groups was compared .Results In the observation group,the cure rate was 95.24%, higher than 80.95%of control group( P <0.05)and the hospitalization time was(12.09 ±3.11)d, shorter than(17.81 ±4.07)d of control group( P <0.01).There was no significant difference between carbon dioxide partial pressures ( P >0.05),however 24 h after treatment,the carbon dioxide partial pressure of observation group was lower than that of the control group ( P <0.01)but oxygen par-tial pressure was higher ( P <0.01).Conclusion The treatment of mechanical ventilation combined with pulmonary surfactant is ob -viously effective to neonates with MAS and can improve blood gas of the patients .
