目的:探讨胎儿超声心动图在产前诊断胎儿先天性心脏病的临床价值。方法选择妊娠>20周,有高危倾向的孕妇及会诊病例共315例行胎儿超声心动图检查。采用胎儿四腔心切面、左右室流出道切面、左室长轴及主动脉根部短轴切面、主动脉弓切面、动脉导管弓切面、三血管切面及上下腔静脉长轴切面、双房及双室短轴切面,观察心脏大小、结构、大血管的位置、走行等;应用彩色多普勒血流显像观察各瓣口及间隔、大血管内血流情况。结果共发现32例先天性心脏病胎儿,其中单心室2例;单心房1例;右心双出口1例,完全性心内膜垫缺损8例,左心发育不良2例;三尖瓣闭锁2例;法洛四联症4例;主动脉瓣狭窄1例;肺动脉瓣狭窄3例;单纯室间隔缺损8例。漏诊房间隔继发孔缺损2例;漏诊肺动脉狭窄1例;敏感性91%,特异性100%。结论胎儿超声心动图是产前诊断胎儿先天性心脏病的最主要的无创性诊断技术,对围产期临床正确处理有重要的指导作用。

목적:탐토태인초성심동도재산전진단태인선천성심장병적림상개치。방법선택임신>20주,유고위경향적잉부급회진병례공315례행태인초성심동도검사。채용태인사강심절면、좌우실류출도절면、좌실장축급주동맥근부단축절면、주동맥궁절면、동맥도관궁절면、삼혈관절면급상하강정맥장축절면、쌍방급쌍실단축절면,관찰심장대소、결구、대혈관적위치、주행등;응용채색다보륵혈류현상관찰각판구급간격、대혈관내혈류정황。결과공발현32례선천성심장병태인,기중단심실2례;단심방1례;우심쌍출구1례,완전성심내막점결손8례,좌심발육불량2례;삼첨판폐쇄2례;법락사련증4례;주동맥판협착1례;폐동맥판협착3례;단순실간격결손8례。루진방간격계발공결손2례;루진폐동맥협착1례;민감성91%,특이성100%。결론태인초성심동도시산전진단태인선천성심장병적최주요적무창성진단기술,대위산기림상정학처리유중요적지도작용。
Objective to investigate the ef ects of fetal echocardiography in the clinical value of prenatal diagnosis of fetal congenital heart disease.Methods choose pregnancy >20 weeks,has a tendency to high -risk pregnant women and 315 consultation cases of routine fetal echocardiography.Using fetal four -chamber view,left and right ventricular outflow tract,left for the final axis and short axis section of the aortic root,aortic arch section,arterial catheter bow section,three vessels on the plane and inferior vena cava long axis section room,double room and two short shaft section,observe the position of the heart size,structure,great vessels,go,etc.;Application of color doppler flow imaging to observe the valve opening and interval,big endovas-cular blood flow. Results 32 cases of congenital heart disease found the fetus,2 of single ventricle;Single atrium in 1 case;Double outlet of right heart in 1 case,completeness,8 cases is of endocardial cushion defect,hypoplastic left heart in 2 cases;Tricuspid atresia in 2 cases;Tetralogy of fal ot 4 cases;Aortic stenosis in 1 case;Pulmonary stenosis (3 cases);8 simple ventricular septal de-fect.Misdiagnosis and interatrial septum secondary hole defect in 2 cases;Misdiagnosis and pulmonary artery stenosis in 1 case;1 0 91% 91% sensitivity and specificity.Conclusion fetal echo-cardiography is the main of non -invasive prenatal diagnosis of fetal congenital heart disease diagnosis technology,the perinatal clinical correctly handle has important guiding role.
