目的：探讨腹腔镜下保留卵巢手术治疗卵巢成熟畸胎瘤蒂扭转的手术疗效及安全性。方法回顾性分析我院2007年～2013年对32例发生蒂扭转的单侧卵巢成熟畸胎瘤行腹腔镜下保留卵巢手术的治疗疗效。结果所有患者均经腹腔镜完成手术,肿瘤大小直径约(7.20±0.60) c m，腹痛时间(16.00±5.47)小时，扭转度数（600.00±184.85）°,术后均无血栓发生，术前和术后3个月F S H和E2水平比较及术后3月阴道B超检测手术侧和未手术侧的窦状卵泡数比较差异均无统计学意义，全部病人随访1年,未见复发，15例未孕患者中有6例术后自然妊娠。结论卵巢良性畸胎瘤蒂扭转患者行腹腔镜下保留卵巢手术是一种安全的手术方式，能很好保留患者卵巢功能。

Objective To investigate the efficacy and long termoutocme of conservative laparoscopic management of ovarian benign teratoma torsion Methods Between 2007 and 2013,32 cases of ovarian benign teratoma torsion removed by conservative laparoscopy were reviewed retrospectivey. Results Al patients completed laparoscopic surgery, the tumor size in diameter (7.20±0.60) cm, abdominal pain, time(16.00±5.47) hours, reversing degrees (600.00±184.85) ° , no postoperative thrombosis, preoperative and intraoperative after three months of FSH and E2 levels and compare the number of antral fol icles after March vaginal B-sides and non-surgical procedure to detect side comparison of the difference was not statistical y significant, al patients were fol owed up for 1 year, no recurrence, 15 cases of nonpregnant there are six cases of postoperative patients with spontaneous pregnancy. Conclusions Laparoscopic conservative surgery is a safe way for Ovarian teratoma torsion patients, patients can be wel retained ovarian function.
