目的:研究无创呼吸机联合纳洛酮治疗老年阻塞性肺病的临床效果。方法选取2010年3月~2013年2月间入院诊治的老年COPD合并呼吸衰竭的患者120例,随机分为实验组(60例)和对照组(60例),其中对照组单纯应用无创呼吸机治疗,实验组在对照组基础上联合纳洛酮治疗。疗程结束后观察两组患者的治疗效果及呼吸、心率、动脉血气指标等。结果实验组显效25例,好转29例,总有效率为90.00%;对照组显效15例,好转30例,总有效率为75.00%。两组患者PaO2、PaCO2、PH、SpO2等动脉血气指标与治疗前相比均有所改善,且实验组疗效更优。上述数据均具有统计学意义(P<0.05)。结论无创呼吸机联合纳洛酮治疗老年阻塞性肺疾病具有良好的疗效,可显著改善动脉血气指标,提高患者的生活质量。

Objective To study the clinical effects of non-invasive ventilation combined with naloxone to elderly patients with obstructive pulmonary disease. Methods 120 elderly COPD patients with respiratory failure hospitalized from March 2010 -February 2013 were randomly divided into experimental group (60 cases) and control group (60 cases). The control group were treated with non-invasive respiratory machine treatment, and the experimental group were based on the combination of both non-invasive respiratory machine and naloxone. Respiration, heart rate, arterial blood gas were utilized as indicators of therapeutic effects of both groups. Results 25 cases with significant effects were observed in experimental group, as wel as 29 cases with relative effects , the total effective rate was 90.00%; 15 cases with significant effects were observed in control group, as wel as 30 cases with relative effects, the total effective rate was 75.00%. PaO2, PaCO2, PH, SpO2 and other indicators of arterial blood gases were al improved in both groups after treatment, with better efficacy in experimental group. Al data were statistical y significant (P <0.05). Conclusion non-invasive ventilation combined naloxone has good efficacy in theating senile obstructive pulmonary disease and in ameliorating arterial blood gas indicators ,, which consequently contributes to better life quality.
