目的：探讨青春期多囊卵巢综合征（ PCOS ）患者超声特征与内分泌指标的相关性。方法选择2011年1月至2013年12月在广州市妇女儿童医疗中心妇科内分泌专科诊治的青春期PCOS患者35例作为病例组，同期选择30例健康青春期女性作为对照组。经直肠超声检查测量卵巢卵泡数、卵巢体积，并检测相关内分泌指标：空腹测血糖（ FPG）、血浆胰岛素（ FINS）、黄体生成素（LH）、卵泡刺激素（FSH）、黄体生成素与卵泡刺激素的比值（LH/FSH）、游离睾酮（FT）、性激素结合球蛋白（SHBG）。结果病例组平均卵巢体积（9．4±3．8 cm3）较对照组（5．1±1．9 cm3）明显增大，差异具有统计学意义（ t＝2．37，P＜0．05）；病例组的FPG、FINS、LH、FT高于对照组，FSH、SHBG低于对照组，差异均具有统计学意义（ tFPG ＝1．94、tFINS ＝2．35、tLH ＝3．14、tFT ＝2．33，P＜0．05；tFSH ＝2．86、tSHBG ＝4．12，P＜0．05）；青春期PCOS患者卵巢卵泡数与LH/FSH呈正相关（r＝0．28，P＜0．05），与FT浓度呈正相关（r＝0．32，P＜0．05）；青春期PCOS患者卵巢体积与LH/FSH呈正相关（r＝0．22，P＜0．05），与FPG浓度呈正相关（r＝0．30，P＜0．05）。结论青春期PCOS患者与健康青春期女性超声特征及内分泌指标具有一定的差异，其超声特征与内分泌指标之间存在一定的相关性。
목적：탐토청춘기다낭란소종합정（ PCOS ）환자초성특정여내분비지표적상관성。방법선택2011년1월지2013년12월재엄주시부녀인동의료중심부과내분비전과진치적청춘기PCOS환자35례작위병례조，동기선택30례건강청춘기녀성작위대조조。경직장초성검사측량란소란포수、란소체적，병검측상관내분비지표：공복측혈당（ FPG）、혈장이도소（ FINS）、황체생성소（LH）、란포자격소（FSH）、황체생성소여란포자격소적비치（LH/FSH）、유리고동（FT）、성격소결합구단백（SHBG）。결과병례조평균란소체적（9．4±3．8 cm3）교대조조（5．1±1．9 cm3）명현증대，차이구유통계학의의（ t＝2．37，P＜0．05）；병례조적FPG、FINS、LH、FT고우대조조，FSH、SHBG저우대조조，차이균구유통계학의의（ tFPG ＝1．94、tFINS ＝2．35、tLH ＝3．14、tFT ＝2．33，P＜0．05；tFSH ＝2．86、tSHBG ＝4．12，P＜0．05）；청춘기PCOS환자란소란포수여LH/FSH정정상관（r＝0．28，P＜0．05），여FT농도정정상관（r＝0．32，P＜0．05）；청춘기PCOS환자란소체적여LH/FSH정정상관（r＝0．22，P＜0．05），여FPG농도정정상관（r＝0．30，P＜0．05）。결론청춘기PCOS환자여건강청춘기녀성초성특정급내분비지표구유일정적차이，기초성특정여내분비지표지간존재일정적상관성。
Objective To study the correlation of ultrasound characteristics and endocrine indexes in adolescent polycystic ovary syndrome (PCOS) patients.Methods Totally 35 adolescent PCOS patients diagnosed in Guangzhou Women and Children ’ Medical Center during the period of January 2011 to December 2013 were recruited in case group , and 30 healthy adolescent girls at the same period were included in control group .The ultrasonic parameters of follicle number and ovarian volume were measured .The serum levels of fasting plasma glucose (FPG), fasting insulin (FINS), luteinizing hormone (LH), follicle stimulating hormone (FSH), the ratio of luteinizing hormone and follicle-stimulating hormone (LH/FSH), free testosterone (FT) and sex hormone binding globulin (SHBG) were tested.Results The ovarian volume of case group (9.4 ±3.8 cm3 ) was significantly higher than the control group (5.1 ±1.9cm3 ), and the difference was significant (t=2.37,P<0.05).The levels of FPG, FINS, LH and FT in case group were all significantly higher than the control group (tFPG =1.94, tFINS =2.35, tLH =3.14,tFT=2.33,all P<0.05), but the levels of FSH and SHBG were significantly lower (tFSH =2.86, tSHBG =4.12, both P <0.05).The follicle number was positively associated with LH/FSH and FT ( r value was 0.28 and 0.32, respectively, both P <0.05).The ovarian volume was positively associated with LH/FSH and FPG (r value was 0.22 and 0.30, respectively, both P<0.05).Conclusion There are differences in ultrasound characteristics and endocrine parameters between adolescent PCOS patients and healthy adolescent girls , and ultrasound characteristics are correlated with endocrine parameters .
