目的:探讨围术期综合呼吸功能训练对肺癌患者术后恢复的效果。方法将82例择期手术治疗的肺癌患者随机分为2组:对照组41例术前术后按胸部手术护理常规给予护理;观察组41例在常规胸部手术护理基础上给予围手术期综合呼吸功能训练。观察2组呼吸系统并发症(肺不张、肺部感染、呼吸衰竭)发生情况;比较2组术前与术后1个月肺功能[最大肺活量(VCmax)、1秒用力呼气容积(FEV1)、FEV1比用力肺活量(FEV1/FVC)及呼吸频率]的变化;记录2组胸导管留置时间及手术至出院的时间。结果观察组术后肺不张、肺部感染、呼吸衰竭发生率均显著低于对照组(P<0.05),肺功能(VCmax、FEV1、FEV1/FVC及呼吸频率)较对照组明显改善(P<0.05),胸导管留置时间与手术至出院时间较对照组明显缩短(P<0.05)。结论通过对肺癌患者实施围术期综合呼吸功能训练可减少患者术后呼吸系统并发症的发生,促进患者术后身体机能恢复,提高其生存质量。

목적:탐토위술기종합호흡공능훈련대폐암환자술후회복적효과。방법장82례택기수술치료적폐암환자수궤분위2조:대조조41례술전술후안흉부수술호리상규급여호리;관찰조41례재상규흉부수술호리기출상급여위수술기종합호흡공능훈련。관찰2조호흡계통병발증(폐불장、폐부감염、호흡쇠갈)발생정황;비교2조술전여술후1개월폐공능[최대폐활량(VCmax)、1초용력호기용적(FEV1)、FEV1비용력폐활량(FEV1/FVC)급호흡빈솔]적변화;기록2조흉도관류치시간급수술지출원적시간。결과관찰조술후폐불장、폐부감염、호흡쇠갈발생솔균현저저우대조조(P<0.05),폐공능(VCmax、FEV1、FEV1/FVC급호흡빈솔)교대조조명현개선(P<0.05),흉도관류치시간여수술지출원시간교대조조명현축단(P<0.05)。결론통과대폐암환자실시위술기종합호흡공능훈련가감소환자술후호흡계통병발증적발생,촉진환자술후신체궤능회복,제고기생존질량。
Objective To investigate the effect of systematic respiratory function training dur-ing the perioperative period on postoperative recovery in patients with lung cancer.Methods Eighty-two patients undergoing elective operation for lung cancer were randomly given routine care alone(control group,n=41)and or in combination with systematic respiratory function train-ing during the perioperative period(observation group,n=41).The incidence of complications(at-electasis,pulmonary infection and respiratory failure)was observed and catheter indwelling time and time from operation to discharge from hospital were recorded in both groups.In addition, maximum vital capacity(VCmax),forced expiratory volume in 1 second(FEV1),FEV1/forced vi-tal capacity(FVC)ration and respiratory rate were determined before and 1 month after opera-tion.Results Compared with control group,the incidence of complications was reduced,pulmona-ry function was improved and catheter indwelling time and time from operation to discharge from hospital were shortened in observation group(P<0.05).Conclusion The systematic respiratory function training during the perioperative period can reduce the incidence of complications,pro-mote postoperative recovery and improve the quality of life in patients with lung cancer.
