目的 本实验以系统性炎症反应综合征(SIRS)为基础,以免疫功能和炎症反应中具有代表性的脾T淋巴细胞为研究对象,观察高压氧(HBO)对SIRS状态机体的影响.方法 健康雄性SD大鼠50只,按数字表法随机分为5组.A、B组:腹腔注射生理盐水(5 ml/kg);C、D、E组:腹腔注射酵母多糖-石蜡悬液(500 mg/kg).D组腹腔注射后4h做1次HBO,B、E组腹腔注射后4、10、18 h各做1次HBO.HBO实施方案:100%纯氧洗舱5 min后匀速加压至0.20 MPa(10 ~ 15 min),维持1h后匀速减压,出舱.所有大鼠予腹腔注射后24h取脾,检测大鼠脾脏T淋巴细胞数量、比例以及脾细胞凋亡率.结果 HBO(B组)降低正常大鼠外周血CD4+T细胞至14.09% (P <0.01),CD8+T细胞基本无变化,脾细胞凋亡有增加,但差异无统计学意义.酵母多糖腹腔注射(C组)使大鼠外周血CD4+、CD8+T细胞平均减少至13.59%及9.75%(P <0.01),脾细胞凋亡率达到25.40% (P <0.05).HBO治疗可使酵母多糖所致CD4+T细胞减少明显恢复,D组为19.68%,E组为18.48%(P<0.01,P<0.05),HBO对CD8+T细胞几乎无影响.HBO治疗组(D、E组)脾细胞凋亡率均明显减少,分别为12.91%、15.97% (P<0.05).结论 HBO可以根据机体处于正常或疾病状态产生截然不同的影响.HBO对正常机体主要是免疫抑制作用,当机体处于炎症状态(SIRS)时,HBO则具有明显的促进免疫功能恢复的作用,并且这种作用可能不依赖HBO的压力和暴露时间.

Objective Based on systematic inflammatory response syndrome (SIRS)and having spleen T-lymphocytes of rats as indicators,which were typical representatives in immune function and process of inflammation,this research aimed at investigating the effects of hyperbaric oxygen (HBO) on normal rats and rats with SIRS.Methods Male healthy SD rats were randomly divided into five groups:the A group,the B groups,the latter group was treated with saline solution (5 ml/kg),the C,D and E groups were treated with zymosan (500 mg/kg).The animals in the D group were given a single session of HBO treatment 4 h after abdominal injection of zymosan,while animals in the B and E groups had HBO exposures respectively at hour 4,10 and 18,following abdominal injection.The HBO treatment profile was as follows:after rinsing of the chamber for 5 min with 100% pure oxygen,the animals were compressed to 2.0 ATA(10 ~ 15 min)at a linear speed,maintained at that pressure for 1 h,then were decompressed to normal pressure and were brought out of the chamber.Spleen samples of all the animals were collected 24 h after abdominal injection.Then,T-lymphocytes counts,ratio and apoptosis of spleen cells were detected.Results For the animals in the B group,HBO exposure could decrease the CD4 + T cell level of the spleen in normal rats to 14.09% (P <0.01),with little changes in CD8 + T.However,there was some increase in apoptosis,but without statistical significance.Abdominal injection of zymosan could decrease the CD4 + and CD8 + T cell level in the peripheral blood of the animals in the C group to 13.59% and 9.75% respectively (P <0.01).Apoptosis rate of spleen cells reached 25.40% (P < 0.05).HBO exposure could significantly promote the reduction of the CD4 + T cell level induced by zymosan,with the animals in the D and E groups recovered to the levels of 19.68% and 18.48% respectively (P< 0.01,P <0.05).HBO seemed to have little effect on CD8+T cells.Rate of apoptosis in all the HBO groups all decreased significantly,declining to the levels of 12.91% and 15.97% respectively(P < 0.05).Conclusions HBO responded differently in accordance with the state of the body.Generally,immunosuppression was the main effect of HBO.When the body was in a state of systematic inflammatory response,HBO had an obvious effect of promoting recovery of immune function,without dependence on the pressure and duration of HBO exposure.
