目的:利用3.0 T磁共振成像技术(MRI)评价扩张型心肌病(DCM)患者的左心结构、功能特征及其对DCM的诊断价值。方法:对45例DCM 患者及30例健康者对照进行 MRI检查,测得左心房及左心室的收缩末期容积、舒张末期容积、收缩末期内径及舒张末期内径,左心室收缩末期及舒张末期球形指数。结果:DCM患者左心室射血分数小于健康对照,其余所有左心结构指标及功能参数都大于健康对照(P<0.05);左心结构与功能参数相关性好,且所有参数诊断DCM的受试者工作特征曲线(ROC)下面积在0.850~1.000之间,当射血分数阈值取33.1%时,敏感度和特异度可达100%。结论:MRI可评价DCM患者左心结构及功能改变,对DCM有很好的诊断价值。

목적:이용3.0 T자공진성상기술(MRI)평개확장형심기병(DCM)환자적좌심결구、공능특정급기대DCM적진단개치。방법:대45례DCM 환자급30례건강자대조진행 MRI검사,측득좌심방급좌심실적수축말기용적、서장말기용적、수축말기내경급서장말기내경,좌심실수축말기급서장말기구형지수。결과:DCM환자좌심실사혈분수소우건강대조,기여소유좌심결구지표급공능삼수도대우건강대조(P<0.05);좌심결구여공능삼수상관성호,차소유삼수진단DCM적수시자공작특정곡선(ROC)하면적재0.850~1.000지간,당사혈분수역치취33.1%시,민감도화특이도가체100%。결론:MRI가평개DCM환자좌심결구급공능개변,대DCM유흔호적진단개치。
Objective:To assess the correlation between the morphologic and functional parameters of left heart using cardiac magnetic resonance imaging (MRI)and its application on dilated cardiomyopathy (DCM).Methods:Forty-five pa-tients with DCM and 30 normal subjects underwent cardiac MRI.The volumes,dimensions and sphericity of the left ventri-cle and atrium in end systolic and end diastolic phase,and ejection fraction were measured.Results:Except that the ejection fraction of DCM were significantly lower than controls,the other parameters of left ventricle and left atrium were signifi-cantly higher than control subjects (P<0.05).There were good correlation between morphologic and functional parameters of left heart,and the parameters might predict DCM (area under receiver operating characteristic curve ROC= 0.850~1.000,P<0.05).Sensitivity and specificity of 100% were achieved for differentiating DCM and controls by using the ejec-tion fraction′s cutoff value of 33.1%.Conclusion:MRI can evaluate the morphological and functional changes of DCM com-pared with the controls,which can be an indicator for diagnosing DCM.
