目的：探讨冷冻切除联合rhTNF-α在恶性脑胶质瘤中的临床应用价值。方法选择颅内胶质瘤患者90例，随机分为两组，每组45例。对照组使用冷冻治疗，观察组在对照组的基础上联合使用rhTNF-α，比较两组患者治疗后新增语言、运动及记忆力障碍比例，治疗不良反应及治疗前及治疗后6个月肿瘤体积的变化。结果观察组治疗后新增语言、运动及记忆力障碍的比例均显著低于对照组（P＜0．05），观察组发生发热和头痛的比例显著低于对照组（P＜0．05）。治疗6个月后，两组肿瘤体积均较治疗前显著缩小（P＜0．05），且观察组小于对照组（P＜0．05），观察组2年生存率高于对照组。结论冷冻治疗联合使用rhTNF-α治疗脑胶质瘤，在减少单纯冷冻治疗不良反应的同时，提高治疗效果延长患者生存时间，值得临床推广。
목적：탐토냉동절제연합rhTNF-α재악성뇌효질류중적림상응용개치。방법선택로내효질류환자90례，수궤분위량조，매조45례。대조조사용냉동치료，관찰조재대조조적기출상연합사용rhTNF-α，비교량조환자치료후신증어언、운동급기억력장애비례，치료불량반응급치료전급치료후6개월종류체적적변화。결과관찰조치료후신증어언、운동급기억력장애적비례균현저저우대조조（P＜0．05），관찰조발생발열화두통적비례현저저우대조조（P＜0．05）。치료6개월후，량조종류체적균교치료전현저축소（P＜0．05），차관찰조소우대조조（P＜0．05），관찰조2년생존솔고우대조조。결론냉동치료연합사용rhTNF-α치료뇌효질류，재감소단순냉동치료불량반응적동시，제고치료효과연장환자생존시간，치득림상추엄。
Objective To investigate the application value of cryoablation combined with rhTNF-αfor malignant glioma.Methods Ninety cases were randomly divided into two groups with 45 cases in each group,the control group were used cryotherapy and the observa-tion group were based on the joint used of rhTNF-α,then new language,movement and memory disorders proportion of adverse reactions and treatment before treatment and 6 months after the change in tumor volume were compared in the two groups.Results After treatment,new language,movement and memory disorders ratio were significantly lower in observation group than those in control group (P<0.05);the oc-currence of fever and headache in observation group were lower than that in control group (P<0.05);after treatment for 6 month,tumor vol-ume of the two groups were smaller than before (P<0.05),and the volume of observation group was smaller than that of control group (P<0.05),2-year survival of observation group was higher than that of control group.Conclusion Cryoablation combined with rhTNF-αfor ma-lignant glioma can reduce the adverse reactions in cryotherapy while increase the therapeutic effect and prolong the survival time,so it is worth clinical promotion.
