目的：探讨膝关节镜下关节清理加外侧支持带松解术治疗膝关节髌骨关节炎的治疗效果。方法对膝关节镜下关节清理加髌骨外侧支持带松解术治疗的膝关节髌股关节炎58例患者临床资料进行回顾分析，其中男性17例，女性41例，左膝18例，右膝38例，双膝2例。患者年龄37～69岁，平均（53．34±3．23）岁。术后均结合康复功能锻炼手术前后随访均按改良Lysholm膝关节功能评分进行临床评价。结果57例膝关节Ⅰ期愈合，1例表浅切口感染，均未发生膝关节血肿；平均随访5．5个月，治疗有效56例，有效率96．55％。参照改良Lysholm疗效评定标准，优49例，良7例，可2例。术前评分为（52．44±10．12）分，术后评分为（88．67±7．13）分，差异有统计学意义（t＝5．93，P＜0．05）。结论膝关节镜下关节清理和髌骨支持带松解术，治疗膝关节髌骨关节炎有较好的临床效果。

목적：탐토슬관절경하관절청리가외측지지대송해술치료슬관절빈골관절염적치료효과。방법대슬관절경하관절청리가빈골외측지지대송해술치료적슬관절빈고관절염58례환자림상자료진행회고분석，기중남성17례，녀성41례，좌슬18례，우슬38례，쌍슬2례。환자년령37～69세，평균（53．34±3．23）세。술후균결합강복공능단련수술전후수방균안개량Lysholm슬관절공능평분진행림상평개。결과57례슬관절Ⅰ기유합，1례표천절구감염，균미발생슬관절혈종；평균수방5．5개월，치료유효56례，유효솔96．55％。삼조개량Lysholm료효평정표준，우49례，량7례，가2례。술전평분위（52．44±10．12）분，술후평분위（88．67±7．13）분，차이유통계학의의（t＝5．93，P＜0．05）。결론슬관절경하관절청리화빈골지지대송해술，치료슬관절빈골관절염유교호적림상효과。
Objective To investigate the effect of arthroscopic debridement under knee arthroscopy and lateral retinaculum release on treating patellofemoral arthritis of knee joint.Methods From 201 2 June to 201 3 Oct,58 patients with patellofemoral arthritis of knee joint who were treated with arthroscopic debridement under knee arthroscopy and lateral retinaculum release were analyzed retrospectively.The age of patients was 37~69 years (53.34 ±3.23 years on average),with 1 7 males and 41 females.Eighteen cases had knee joint patellofemoral arthritis in left,38 cases in right and 2 cases in both.Arthroscopic debridement under knee arthroscopy and lateral retinaculum release were used,combined with CPM machine and postoperative knee joint functional exercise of rehabilitation in analgesia.Results Fifty-seven cases had knee joint primary healing.One case had infection in surface incisional wound.No knee joint hematoma occurred.The average follow-up time was 5.5 months.56 cases had effects,and the effective rate was 95.6%.With reference to the improved Lysholm therapeutic effect evalu-ation criteria,49 cases were exellent,7 cases were good,and 2 cases were tolerable.The average score of pre-operative was (52.44 ±1 0.1 2) and that of post-operative was (88.67 ±7.1 3),with significant difference by paired t test (t=5.93,P<0.05).Conclusion After arthro-scopic debridement under knee arthroscopy and lateral retinaculum release,there is pretty good clinical effect on patellofemoral arthritis of knee joint patients with obvious front knee patellar pain symptoms.
