目的:了解血友病患者口腔健康状况并探讨其相关影响因素。方法2011年2月-2012年9月,对179例血友病患者进行口腔保健知识及行为的问卷调查,并由牙周病专业医生对患者进行口腔检查,包括菌斑指数、牙石指数、出血指数及龋失补指数等口腔健康状况指标。采用多因素分析建回归模型的方法,分析血友病患者口腔健康状况的影响因素。结果口腔保健行为问卷得分及牙石指数与菌斑指数显著相关(P<0.05);患者的年龄、龋失补指数、自发牙龈出血、出血指数和菌斑指数与牙石指数显著相关(P<0.05);年龄、凝血因子含量、菌斑指数、龋失补指数和牙石指数与出血指数显著相关(P<0.05);日常吃软物出血、牙石指数、医保分类与龋失补指数显著相关(P<0.05)。结论口腔保健行为是血友病患者口腔健康程度的重要影响因素,菌斑的发生进一步影响牙石的产生,又进行性发展为牙龈出血,吃软物出血则是患者高发龋失补的重要标志。
目的:瞭解血友病患者口腔健康狀況併探討其相關影響因素。方法2011年2月-2012年9月,對179例血友病患者進行口腔保健知識及行為的問捲調查,併由牙週病專業醫生對患者進行口腔檢查,包括菌斑指數、牙石指數、齣血指數及齲失補指數等口腔健康狀況指標。採用多因素分析建迴歸模型的方法,分析血友病患者口腔健康狀況的影響因素。結果口腔保健行為問捲得分及牙石指數與菌斑指數顯著相關(P<0.05);患者的年齡、齲失補指數、自髮牙齦齣血、齣血指數和菌斑指數與牙石指數顯著相關(P<0.05);年齡、凝血因子含量、菌斑指數、齲失補指數和牙石指數與齣血指數顯著相關(P<0.05);日常喫軟物齣血、牙石指數、醫保分類與齲失補指數顯著相關(P<0.05)。結論口腔保健行為是血友病患者口腔健康程度的重要影響因素,菌斑的髮生進一步影響牙石的產生,又進行性髮展為牙齦齣血,喫軟物齣血則是患者高髮齲失補的重要標誌。
목적:료해혈우병환자구강건강상황병탐토기상관영향인소。방법2011년2월-2012년9월,대179례혈우병환자진행구강보건지식급행위적문권조사,병유아주병전업의생대환자진행구강검사,포괄균반지수、아석지수、출혈지수급우실보지수등구강건강상황지표。채용다인소분석건회귀모형적방법,분석혈우병환자구강건강상황적영향인소。결과구강보건행위문권득분급아석지수여균반지수현저상관(P<0.05);환자적년령、우실보지수、자발아간출혈、출혈지수화균반지수여아석지수현저상관(P<0.05);년령、응혈인자함량、균반지수、우실보지수화아석지수여출혈지수현저상관(P<0.05);일상흘연물출혈、아석지수、의보분류여우실보지수현저상관(P<0.05)。결론구강보건행위시혈우병환자구강건강정도적중요영향인소,균반적발생진일보영향아석적산생,우진행성발전위아간출혈,흘연물출혈칙시환자고발우실보적중요표지。
Objective To explore factors correlating with oral health of hemophiliac. Methods From Feb. 2011 to Sept. 2012, oral health knowledge and behaviors in patients with severe hemophilia were surveyed by questionnaire at hemophilia treatment center of Peking Union Medical College Hospital, and oral examinations were performed by periodontists. Oral health evaluation indexes including dental plaque index, calculus index, bleeding index and decayed, missing and filled teeth (DMFT) indexes were recorded. Results Both the behavior questionnaire score and calculus index were significantly correlated with plaque index ( P<0.05) and patient’s age and education, and plaque index were significantly correlated with calculus index (P<0.05). Both the behavior questionnaire score and calculus index were significantly correlated with bleeding index ( P<0.05) and bleeding during eating soft food was significantly correlated with DMFT (P<0.05). Conclusion Oral health behavior is an important factor affecting oral health of hemophiliacs. The occurrence of dental plaques can affect the generation of calculus, which further develops progressively into gingival bleeding. Bleeding during eating soft food is an important sign of high incidence of DMFT (decaying, missing and filling of teeth).
