目的：探讨采用穴位自血疗法联合痤疮散外敷治疗痤疮的作用机理及对肿瘤坏死因子-α（TNF-α）及患者血清中白细胞介素8（IL-8）的影响。方法将本院2012年11月～2013年10月皮肤科收治的180例面部痤疮患者随机分为观察组、对照1组、对照2组，每组60例；另选择健康体检者50例为对照3组。观察组患者采用穴位自血疗法及痤疮散面部外敷治疗痤疮；对照1组采用穴位自血疗法；对照2组给予罗红霉素胶囊口服及阿达帕林凝胶外用；对照3组未做任何治疗。观察痤疮治疗效果及治疗前、治疗后第4周、第8周、第12周三组患者皮损状态、皮损计数；TNF-α与IL-8指标。结果观察组痊愈率、总有效率明显优于对照组，差异有显著意义（P＜0．05）；观察组治疗前、治疗后第4周、第8周、第12周，平均皮损数量减少与对照组比较差异有显著意义（ P＜0．05）；观察组TNF-α与IL-8治疗前、治疗后第4周、第8周、第12周与对照组比较，差异有显著意义（P＜0．05）。结论采用穴位自血疗法结合痤疮散外敷治疗面部痤疮并实施专业护理，能有效促进痤疮恢复，且总有效率高。TNF-α和IL-8均参与了痤疮的发病过程，与痤疮的严重程度成正比，在痤疮皮损的形成中起着重要作用。随着用药时间的增长，TNF-α和IL-8明显降低，说明该联合治疗法对痤疮有明显疗效。
목적：탐토채용혈위자혈요법연합좌창산외부치료좌창적작용궤리급대종류배사인자-α（TNF-α）급환자혈청중백세포개소8（IL-8）적영향。방법장본원2012년11월～2013년10월피부과수치적180례면부좌창환자수궤분위관찰조、대조1조、대조2조，매조60례；령선택건강체검자50례위대조3조。관찰조환자채용혈위자혈요법급좌창산면부외부치료좌창；대조1조채용혈위자혈요법；대조2조급여라홍매소효낭구복급아체파림응효외용；대조3조미주임하치료。관찰좌창치료효과급치료전、치료후제4주、제8주、제12주삼조환자피손상태、피손계수；TNF-α여IL-8지표。결과관찰조전유솔、총유효솔명현우우대조조，차이유현저의의（P＜0．05）；관찰조치료전、치료후제4주、제8주、제12주，평균피손수량감소여대조조비교차이유현저의의（ P＜0．05）；관찰조TNF-α여IL-8치료전、치료후제4주、제8주、제12주여대조조비교，차이유현저의의（P＜0．05）。결론채용혈위자혈요법결합좌창산외부치료면부좌창병실시전업호리，능유효촉진좌창회복，차총유효솔고。TNF-α화IL-8균삼여료좌창적발병과정，여좌창적엄중정도성정비，재좌창피손적형성중기착중요작용。수착용약시간적증장，TNF-α화IL-8명현강저，설명해연합치요법대좌창유명현료효。
Objective To study the mechanism of autoblood point injection therapy combined with acne powder for the treatment of tumor necrosis factor alpha (TNF-α) and effect on serum interleukin 8 (IL-8) .Method 180 pa-tients with facial acne from 2012 November to 2013 October admitted to Department of Dermatology were selected and randomly divided into observation group ,control group 1 and control group 2 .60 cases in each group .Observa-tion group were treated with acupuncture Autohemotherapy and external application of acne powder for facial acne treatment .Patients in control group 1 was given acupoint Autohemotherapy .Patients in control group 2 was given orally 2 roxithromycin capsules and topical adapalene gel .Another 50 cases of healthy people without making any treatment was selected as control group 3 .The therapeutic effect of acne ,patients with lesions ,and lesions counts of TNF-α and IL-8 index .was measured before treatment ,after fourth weeks ,eighth weeks ,Twelfth weeks in three groups .Result In the observation group ,the cure rate and total effective rate was better than that of control group ,There was significant difference(P< 0 .05) .The average number of lesions in observation group was de-creased before treatment ,after fourth weeks ,eighth weeks ,Twelfth weeks compared with control group .There was significant difference (P<0 .05) .The TNF-α and IL-8 in observation group before treatment ,after fourth weeks ,eighth weeks ,,twelfth weeks was significant difference compared with control group (P<0 .05) .Conclusion Using autoblood point injection therapy combined with acne powder for the treatment of facial acne and the imple-mentation of professional nursing ,can effectively promote the recovery of acne ,and the total effective rate .TNF-αand IL-8 are involved in the pathogenesis of acne ,proportional to the severity of acne ,plays an important role in the formation of acne lesions ,in addition ,along with the time growth ,TNF-αand IL-8 decreased significantly ,which indicated that the combined therapy has obvious curative effect on .
