目的:通过对腹泻症候群中致泻大肠埃希菌的血清型和多重PCR检测,了解本地区致泻大肠埃希菌的血清群、型分布规律,所携带毒力基因及相互之间的相关性,为腹泻症候群中致泻大肠埃希菌的检测、鉴定提供科学依据,以提高阳性株的检出率。方法:对本地区2012-2013年腹泻症候群监测医院门诊未使用过抗生素腹泻患者的稀便、水样便、黏脓便或脓血便标本通过増菌、各种选择性培养基筛选出病原菌,后经生化试验、多重PCR试验和血清型分型试验进行鉴定。结果:本次检测共检出65株血清凝集致泻大肠埃希菌,分属EPEC、EIEC、ETEC,以EPEC为主,占83.08%,共8个血清型,其中血清型O55:H59、O128:H67占58.47%;共检出4种相关毒力基因,其中escV 49株(75.38%)。未分血清型菌株27株,检出相关毒力基因共5种,分属EPEC、EIEC、ETEC、EAEC,其中escV 15株(55.56%)。结论:本地区腹泻症候群中血清学阳性致泻大肠埃希菌以EPEC为主,常见血清型为O55:H59、O128:H67,毒力基因为escV、escV+bfpB、invE、elt。未分血清型致泻大肠埃希菌毒力基因为escV、escV+bfpB、invE、elt、astA,与已分型菌株携带基因基本一致。腹泻患者的大肠埃希菌检测中,在传统的细菌分离培养、生化试验、血清学鉴定的基础上,有必要进行大肠埃希菌的毒力基因检测,以提高阳性检出率,避免漏检,提高致泻性大肠埃希菌的诊断。

목적:통과대복사증후군중치사대장애희균적혈청형화다중PCR검측,료해본지구치사대장애희균적혈청군、형분포규률,소휴대독력기인급상호지간적상관성,위복사증후군중치사대장애희균적검측、감정제공과학의거,이제고양성주적검출솔。방법:대본지구2012-2013년복사증후군감측의원문진미사용과항생소복사환자적희편、수양편、점농편혹농혈편표본통과증균、각충선택성배양기사선출병원균,후경생화시험、다중PCR시험화혈청형분형시험진행감정。결과:본차검측공검출65주혈청응집치사대장애희균,분속EPEC、EIEC、ETEC,이EPEC위주,점83.08%,공8개혈청형,기중혈청형O55:H59、O128:H67점58.47%;공검출4충상관독력기인,기중escV 49주(75.38%)。미분혈청형균주27주,검출상관독력기인공5충,분속EPEC、EIEC、ETEC、EAEC,기중escV 15주(55.56%)。결론:본지구복사증후군중혈청학양성치사대장애희균이EPEC위주,상견혈청형위O55:H59、O128:H67,독력기인위escV、escV+bfpB、invE、elt。미분혈청형치사대장애희균독력기인위escV、escV+bfpB、invE、elt、astA,여이분형균주휴대기인기본일치。복사환자적대장애희균검측중,재전통적세균분리배양、생화시험、혈청학감정적기출상,유필요진행대장애희균적독력기인검측,이제고양성검출솔,피면루검,제고치사성대장애희균적진단。
Objective:To detect the serum type of diarrhea caused by Escherichia coli diarrhea syndrome and multiple PCR,understand the diarrhea caused by Escherichia coli serogroups,region based distribution,the correlation between virulence genes and their detection,identification,and provide scientific basis for diarrhea syndrome caused by large Escherichia coli,in order to improve the detection rate of positive strains.Method:The local diarrhea syndromic surveillance hospital unused,watery,sticky pus or purulent blood sample diluted by those bacteria,a variety of selective culture medium screening pathogen antibiotics in patients with diarrhea from 2012 to 2013 ,after biochemical test, multiple PCR test and serotype tests were identified.Result:65 strains serum agglutination of diarrheogenic Escherichia coli were detected and belong to EPEC,EIEC,ETEC,the EPEC accounted for 83.08%,a total of 8 serotypes,serotype O55:H59,O128:H67,which accounted for 58.47%;there were 4 kinds of virulence related genes,among them 49 were escV(75.38%). Serum without typing were 27 strains,5 species of virulence related genes were detected,which escV had 15 strain(55.56%),belong to EPEC,EIEC,ETEC,EAEC.Conclusion:Local serological diarrhea syndrome is caused by Escherichia coli EPEC,common serotypes are O55:H59,O128:H67;virulence genes are escV,escV+bfpB, invE,elt. Serum without typing diarrheogenic Escherichia coli virulence genes are escV,escV+bfpB,invE,elt,astA, and consistent with typing of strains carrying genes. Detection of Escherichia coli diarrhea patients,basal medium,in bacterial isolation and conventional biochemical tests,serological identification,virulence gene detection is necessary to Escherichia coli,in order to improve the positive rate,to avoid detection,improve the diagnosis of diarrhea genic Escherichia coli.
