目的：探讨应用微型钛板结合空心双头加压螺钉内固定治疗MasonⅢ型桡骨小头骨折的可行性及优点。方法：选取2008年3月-2013年5月笔者所在医院收治的55例MasonⅢ型桡骨小头骨折手术患者，均应用微型T形锁定钛板结合空心双头加压螺钉内固定，术后根据X线片表现、肘关节活动度及Broberg and Morrey功能评分等进行疗效评定。结果：所有病例骨折均愈合，愈合时间8~15周，平均10.2周；无桡骨头坏死发生；手术时间64~108 min，平均80.2 min；术后随访6~48个月，平均24.6个月。肘关节屈曲125°~135°，平均132.2°；伸直0°，前臂旋前73°~90°，平均85.4°；旋后70°~90°；平均81.2°。Broberg and Morrey肘关节评分平均91.6分，优24例，良23例，可8例，优良率85.5%。结论：应用微型钛板结合空心双头加压螺钉内固定治疗MasonⅢ型桡骨小头骨折，有固定坚强、利于早期功能锻炼、利于骨折愈合、功能恢复好、并发症少等优点，是一种较实用的手术方法。
목적：탐토응용미형태판결합공심쌍두가압라정내고정치료MasonⅢ형뇨골소두골절적가행성급우점。방법：선취2008년3월-2013년5월필자소재의원수치적55례MasonⅢ형뇨골소두골절수술환자，균응용미형T형쇄정태판결합공심쌍두가압라정내고정，술후근거X선편표현、주관절활동도급Broberg and Morrey공능평분등진행료효평정。결과：소유병례골절균유합，유합시간8~15주，평균10.2주；무뇨골두배사발생；수술시간64~108 min，평균80.2 min；술후수방6~48개월，평균24.6개월。주관절굴곡125°~135°，평균132.2°；신직0°，전비선전73°~90°，평균85.4°；선후70°~90°；평균81.2°。Broberg and Morrey주관절평분평균91.6분，우24례，량23례，가8례，우량솔85.5%。결론：응용미형태판결합공심쌍두가압라정내고정치료MasonⅢ형뇨골소두골절，유고정견강、리우조기공능단련、리우골절유합、공능회복호、병발증소등우점，시일충교실용적수술방법。
Objective:To explore the advantages of the micro titanium plate and the double-head hollow compression screw in internal fixation of MasonⅢradial head fractures.Method:From March 2008 to May 2013,55 cases of MasonⅢradial head fracture were treated with the Micro locking titanium plate and the double-head hollow compression screw. The outcomes were assessed in regard of range of motion and the Broberg and Morrey elbow performance score system. Result:All the fractures healed,the healing time of 8-15 weeks,an average of 10.2 weeks;no necrosis radial head;operation time 64-108 minutes,mean 80.2 minutes;patients were followed up for 6-48 months,an average of 24.6 months.Elbow flexion 125°-135°,an average of 132.2°;Straight 0°,forearm pronation 73°-90°,an average of 85.4°;Spin after 70°-90°;an average of 81.2°. Broberg and Morrey elbow score averaged 91.6 points,the result were excellent in 24 cases,good in 23 cases,common in 8 case,the excellent and good rate was 85.5%.Conclusion:Treatment of Mason type Ⅲradial head fractures by internal fixation with the Micro locking titanium plate and the double-head hollow compression screw is a practical method with the advantages of rigid fixation,early exercise,favorable to the healing of the fracture,better elbow function,less complication.
