目的 总结中枢神经细胞瘤的血供特点,探讨其对手术治疗的指导意义.方法 回顾性分析27例中枢神经细胞瘤患者的脑血管造影检查,总结其动脉期、毛细血管期及静脉期表现,结合手术入路分析其临床意义.结果 肿瘤染色程度与其体积呈正相关,供血动脉最常见为脉络膜后内侧及外侧动脉,肿瘤引流静脉为Galen静脉系统,同侧丘纹静脉被肿瘤扭曲抬高,同侧的大脑内静脉和大脑大静脉被压向下外方,静脉角不规则扩张.结论 手术全切除是中枢神经细胞瘤最佳治疗手段.脑血管造影检查有助于选择正确手术入路、减少术中出血、防止副损伤、提高肿瘤的全切除率.

Objective To analyze the angiographic characteristics of central neurocytomas and discuss their significance to surgical strategies.Method The angiographic characteristics of 27 cases of central neurocytoma were retrospectively analyzed.Their surgical procedures were reviewed and cerebral angiographies were thoroughly examined in arterial,capillary and venous phases.Results The degree of tumor - staining was related to the size of tumor.These tumors commonly received their blood supply from the medial and lateral posterior choroidal arteries.The enlarged ipsilateral thalamostriate veins were elevated upward and the ipsilateral internal cerebral veins were depressed downward in the lateral projection of the carotid angiography.The venous angle were irregularly widened.Conclusions Surgery should be the best strategy for central neurocytomas.The angiography should be listed as the essential examination so that the eloquent structure could be protected and the GTR rate is improved.
