目的：探讨高 b值扩散加权成像(DWI)在颅内 DWI 高信号病变的临床应用价值。方法回顾性分析24例颅内 DWI高信号病变患者3.0T MR图像及数据,比较常规 b值(b=1000 s/mm2)与高 b值(b=3000 s/mm2)DWI对病变的显示能力及诊断价值。结果常规 b值及高 b值DWI图像的信噪比为318.4±26.3和143.7±42.6(P<0.05)。常规 b 值及高 b 值 DWI 图像的对比度分别为0.042±0.012和0.178±0.027(P<0.05)。高 b值相比较常规 b 值 DWI 图像的信噪比下降,但对病变边界显示更清晰、定位更准确、可发现的病灶数量增加。结论高 b值DWI对多发性脑转移瘤的诊断优于常规 DWI,可作为脑转移瘤的检查项目开展。

Objective To evaluate the clinical application value of diffusion weighted MR imaging(DWI)with high b value in cere-bral diseases which were high signal intensity on DWI.Methods MRI findings data of 24 patients with cerebral disease at 3.0T MR scanner were retrospectively analyzed,the displaying ability and diagnostic value of DWI with b values of 1 000 s/mm2 and 3 000 s/mm2 were compared.Results Signal to noise ratio of DWI was 318.4±26.3 with routine b value and 143.7±42.6 with high b value respectively (P<0.05).Image contrast was 0.042±0.012 on DWI with routine b value and 0.178±0.027 on DWI with high b value respectively (P<0.05),compared with DWI with routine b value,the boundary of lesions was displayed clearer on DWI with high b value,the location of lesions was more precise on DWI with high b value,and the number of lesions had been found was more on DWI with high b value.Conclusion DWI with high b value is more superior than which with routine b value on diagnosis of multiple brain me-tastasis,and could be applied as a inspection proj ect of brain metastasis.
