目的:探讨乙二胺四乙酸(EDTA)依赖性假性血小板减少的临床解决思路。方法采集5例 EDTA依赖性假性血小板减少患者的 EDTA 及枸橼酸钠抗凝血,每位患者的 EDTA-K2抗凝血均在不同时间段分别加入6.5 mg/mL 阿米卡星,并依次采用血液分析仪检测及血涂片检查。结果3例患者在抽血前或抽血后1.5 h 内在其 EDTA 抗凝血中加入阿米卡星后能在不影响其它血细胞形态和分布的情况下解离凝集血小板,其血小板计数能在室温下4 h 之内维持稳定。余下2例患者,加或不加阿米卡星,其血小板数均会随着时间的延长逐渐增加,最终基本恢复正常水平,阿米卡星起到加速作用。结论添加阿米卡星的血小板检测结果在4 h 保持稳定,结果明显优于更换枸橼酸钠抗凝剂。该药在医院抗菌药物中使用普遍,且在临床实际工作中,可以减少患者重复采血,缩短报告等候时间。阿米卡星可作为处理 EDTA 依赖的假性血小板减少的一线方法在临床普及。

목적:탐토을이알사을산(EDTA)의뢰성가성혈소판감소적림상해결사로。방법채집5례 EDTA의뢰성가성혈소판감소환자적 EDTA 급구연산납항응혈,매위환자적 EDTA-K2항응혈균재불동시간단분별가입6.5 mg/mL 아미잡성,병의차채용혈액분석의검측급혈도편검사。결과3례환자재추혈전혹추혈후1.5 h 내재기 EDTA 항응혈중가입아미잡성후능재불영향기타혈세포형태화분포적정황하해리응집혈소판,기혈소판계수능재실온하4 h 지내유지은정。여하2례환자,가혹불가아미잡성,기혈소판수균회수착시간적연장축점증가,최종기본회복정상수평,아미잡성기도가속작용。결론첨가아미잡성적혈소판검측결과재4 h 보지은정,결과명현우우경환구연산납항응제。해약재의원항균약물중사용보편,차재림상실제공작중,가이감소환자중복채혈,축단보고등후시간。아미잡성가작위처리 EDTA 의뢰적가성혈소판감소적일선방법재림상보급。
Objective To study on the laboratory solutions for ethylenediaminetetraacetic acid (EDTA)-dependent pseudothrombocytopenia.Methods A total of 5 patients with EDTA-dependent pseudothrombocytopenia were enrolled, and each blood with EDTA-K2 anticoagulant was added to 6.5 mg/mL amikacin in different time periods.The blood routine test and blood smears were performed.Results In 3 patients before collecting blood or collecting blood after 1.5h,its inherent EDTA anticoagulant was added amikacin,which can not affect the situation of other blood cell morphology and distribution of dissociation platelet aggregation,and the platelet count can be stable within 4h at room temperature.In the other 2 patients,with or without amikacin,the platelet count increased gradually with time and eventually returned to normal levels,and amikacin played acceleration role.Conclusions The test results of platelet adding amikacin in 4h are stable and reliable,and the drug use of antibiotics in hospitals is general.In clinical practice,it can reduce the duplication of collecting blood and shorten turn-around-time.Amikacin should be as first-line approach in the treatment of EDTA-dependent pseudothrombocytopenia.
