目的 探讨累及黄斑区的孔源性视网膜脱离(RRD)患者行巩膜扣带手术后频域光学相干断层扫描(OCT)检测的黄斑中心凹视网膜厚度变化及中心视野的改变.方法 临床病例分析研究.对2012年3~12月间在上海交大附属第六人民医院眼科就诊的累及黄斑区的孔源性视网膜脱离患者16例(16只眼)行巩膜外加压术.术前及术后1、3、6个月所有患者均行最佳矫正视力(BCVA)、采用Octopus101自动视野计行视野检查、应用频域OCT对黄斑区结构进行检测.观察黄斑中心凹视网膜厚度变化,分析黄斑中心凹厚度与最佳矫正视力、视野光敏感度的相关性.结果 术前与术后1、3、6个月患者黄斑中心凹厚度分别为(520.19±41.63)μm、(328.81±90.74) μm、(279.06±57.90)μ m、(261.75±47.13) μ m,术后黄斑中心凹厚度较术前明显降低差异均有统计学意义(P <0.05); BCVA分别为0.03±0.01、0.26±0.13、0.47±0.17、O.48±0.16,术后BCVA较术前明显提高差异均有统计学意义(P<0.05);4°视野平均敏感度(MS)分别为(13.49±0.92)dB、(21.05±3.25) dB、(22.17±2.01)dB、(22.91±1.85) dB,术后MS较术前明显提高差异均有统计学意义(P<0.05).术后1、3、6个月患者黄斑中心凹厚度与BCVA呈中度负相关(r=-0.731、-0.749、-0.629,P<0.05),与MS呈高度负相关(r=-0.904、-0.885、-0.821,P<0.05).结论 Octopus101自动视野计检测的中心4°视野MS能准确地反映RRD术后黄斑视功能,与频域OCT检测的黄斑中心凹厚度有良好相关性.

Objective To explore macular foveal thickness changes measured with spectral domain optical coherence tomography (OCT) and central visual field changes in patients with rhegmatogenous retinal detachment (RRD) involving the macular area after scleral buckling surgery.Methods Sixteen patients (16 eyes) with RRD involving the macular area underwent scleral buckling surgery.Best corrected visual acuity (BCVA) check,visual field examination by using Octopusl01 automated perimetry and OCT examination for observation foveal retinal thickness were performed before and 1,3,6 months after surgery.The foveal retinal thickness was compared.The correlations between macular foveal thickness and BCVA,visual field sensitivity were analyzed.Results Preoperative and postoperative 1,3,6 months,the foveal retinal thickness were (520.19± 41.63)μm,(328.81±90.74)μm,(279.06 ±57.90)μm,(261.75± 47.13)μ m,respectively,the foveal retinal thickness after surgery decreased significantly,the differences were statistically significant (all P<0.05); BCVA were (0.03±0.01),(0.26±0.13),(0.47±0.17),(0.48±0.16),respectively,BCVA after surgery significantly improved,the differences were statistically significant (all P <0.05); 4° visual field mean sensitivity (MS) were (13.49 ± 0.92)dB,(21.05±3.25)dB,(22.17±2.01)dB,(22.91±1.85) dB,respectively,MS after surgery significantly increased,the differences were statistically significant (all P<0.05).The foveal thickness and BCVA in patients showed moderate negative correlation (r=-0.731,-0.749,-0.629,P <0.05),the foveal thickness and MS showed high negative correlation (r=-0.904,-0.885,-0.821,P <0.05) 1,3,and 6 months postoperatively.Conclusions 4° visual field MS detected by using the Octopusl01 automated perimetry can accurately reflect macular visual function RRD postoperative,there is a good correlation between foveal thickness measured with spectral domain OCT and 4° visual field MS.
