目的:通过测定新生儿脐血中肌酸激酶(CK)、肌酸激酶同工酶MB(CK-MB)和脑钠肽(BNP)的水平,结合脐血乳酸的结果,研究新生儿脐血乳酸与心肌酶的关系。方法收集2009年7~12月在中山大学附属第一医院产房分娩的新生儿65例,分别测定脐动静血的pH值、剩余碱等指标及脐静脉血乳酸、BNP和心肌酶(CK-MB、CK),并对上述指标进行相关性分析。结果血乳酸水平与血气分析各项指标具有密切的统计学负相关性。脐动脉血气分析结果与心肌酶及BNP无明显统计学关联,血乳酸水平与CK-MB具有统计学正相关性,而与CK及BNP并无统计学关联性。结论乳酸与血气分析指标中的pH值、剩余碱有密切的关联性,可以在缺氧早期即反映出机体的酸碱平衡状态,而相较于血气分析,更能够特异性地衡量由于代谢性酸中毒造成的心肌损伤情况,与CK-MB线性相关,可作为判断心肌损伤发生、预测严重程度的指标。
目的:通過測定新生兒臍血中肌痠激酶(CK)、肌痠激酶同工酶MB(CK-MB)和腦鈉肽(BNP)的水平,結閤臍血乳痠的結果,研究新生兒臍血乳痠與心肌酶的關繫。方法收集2009年7~12月在中山大學附屬第一醫院產房分娩的新生兒65例,分彆測定臍動靜血的pH值、剩餘堿等指標及臍靜脈血乳痠、BNP和心肌酶(CK-MB、CK),併對上述指標進行相關性分析。結果血乳痠水平與血氣分析各項指標具有密切的統計學負相關性。臍動脈血氣分析結果與心肌酶及BNP無明顯統計學關聯,血乳痠水平與CK-MB具有統計學正相關性,而與CK及BNP併無統計學關聯性。結論乳痠與血氣分析指標中的pH值、剩餘堿有密切的關聯性,可以在缺氧早期即反映齣機體的痠堿平衡狀態,而相較于血氣分析,更能夠特異性地衡量由于代謝性痠中毒造成的心肌損傷情況,與CK-MB線性相關,可作為判斷心肌損傷髮生、預測嚴重程度的指標。
목적:통과측정신생인제혈중기산격매(CK)、기산격매동공매MB(CK-MB)화뇌납태(BNP)적수평,결합제혈유산적결과,연구신생인제혈유산여심기매적관계。방법수집2009년7~12월재중산대학부속제일의원산방분면적신생인65례,분별측정제동정혈적pH치、잉여감등지표급제정맥혈유산、BNP화심기매(CK-MB、CK),병대상술지표진행상관성분석。결과혈유산수평여혈기분석각항지표구유밀절적통계학부상관성。제동맥혈기분석결과여심기매급BNP무명현통계학관련,혈유산수평여CK-MB구유통계학정상관성,이여CK급BNP병무통계학관련성。결론유산여혈기분석지표중적pH치、잉여감유밀절적관련성,가이재결양조기즉반영출궤체적산감평형상태,이상교우혈기분석,경능구특이성지형량유우대사성산중독조성적심기손상정황,여CK-MB선성상관,가작위판단심기손상발생、예측엄중정도적지표。
ObjectiveTo explore the relationship between neonatal cord blood lactate and myocardial enzymes. MethodsThis study recruited 65 neonates delivered in our hospital from July 2009 to December 2009. To detect their cord blood CK/CK-MB/BNP and cord blood lactate, pH and BE. To statistics them by correlation.ResultsCord blood lactate was negatively correlated with many markers in blood gas analysis. No significant difference was found in umbilical artery blood gas result and any of the myocardial enzymes. Cord blood lactate was positively correlated with cord blood CK-MB, however, not correlated with cord blood CK and BNP.ConclusionsCord blood lactate could reflect body’s acid-base balance situation in early phase of hypoxia. And it is more specific than blood gas analysis in judgment of myocardial Injury. Cord blood lactate was positively correlated with cord blood CK-MB, so it might predict the severity of myocardial injury, as a useful marker.
